Intracranial vascular complications following radiosurgery are extremely rare.We report a case of stenosis in the internal carotid artery 5 years after gamma knife for recurrent pituitary adenoma. Percutaneous transluminal angioplasty was performed successfully with anatomical and functional improvement.These results suggested importance monitoring arterial long-term follow-up. Moreover, this is first endovascular treatment as an effective therapy intracranial due to radiotherapy.

A 79-year-old woman presented with a subarachnoid hemorrhage. Angiography revealed pseudoaneurysm formation due to rupture of true saccular anterior communicating artery aneurysm. Coil embolization, limited the aneurysm, was performed successfully favorable clinical outcome. This procedure can be considered as an alternative treatment option for similar aneurysms in cases where surgical clipping is contraindicated.
症例は, 無症候のため半年間経過観察された慢性硬膜下血腫 (chronic subdural hematoma : CSDH) の81歳男性. ADL低下と発熱を主訴に来院した. 入院し熱源精査中の5日目に左片麻痺をきたし, 既存血腫に対し穿頭術を施行した. 血液の混じった膿汁が排液され, 感染性硬膜下血腫 (infected ISH) と診断した. その後再発し, 穿頭術を2回施行した. 抗菌薬で炎症反応も軽快し, 病変は画像上器質化したと判断された. 発熱と炎症反応を伴うCSDHはISHの可能性があり注意が必要である. 診断および治療に苦慮した経過の詳細を記載し, 過去の報告例を分析した.
