To investigate the relationship between exposure and efficacy of tolvaptan, we measured pharmacokinetics total drug at 7 days after repeated doses 3.75 mg/day tolvaptan in 16 patients with hepatic edema. Nine (56.3%) were responders, which defined as those body weight reduction >1.5 kg/week. Serum albumin levels significantly lower responders than non-responders (P = 0.031). 症例は49歳,女性.背部痛を主訴に受診.造影CTで膵頭部に乏血性の腫瘤を認め,肝臓にも両葉多発の腫瘤を認めた.超音波内視鏡下穿刺吸引法による組織診の結果,退形成膵管癌と診断.化学療法目的に当院へ紹介となり80%doseのgemcitabine+nab-paclitaxelにて治療開始.治療後3か月で画像上,原発巣のわずかな縮小と肝転移巣の著明な縮小を認めたが, 食思不振とperformance status低下にて治療の継続は困難となった.S-1内服に変更するも急激なCA19-9の上昇と全身衰弱にて初診より約4か月で永眠された.病理解剖の結果,多形細胞型退形成膵管癌との診断となった.化学療法を施行した多型細胞型退形成膵管癌の報告は少なく,画像上奏功した症例はほとんどない.本症例は,切除不能多形細胞型退形成膵管癌の治療を検討する上で貴重な症例と考えた.

症例は65歳,男性.健診でCA19-9高値を指摘されて受診した.CTで膵頭部に5.0cm大の乏血性腫瘍を認め,EUS-FNAにより膵癌と診断した.腹腔動脈に進展し門脈に浸潤を認めたことから切除不能な局所進行膵癌であり,gemcitabine(GEM),S-1を併用した化学放射線療法を施行し,その後はGEM単剤で化学療法を継続した.治療開始から1年4ヶ月後には原発巣は指摘困難な程度まで縮小し,2年後のPETでは同部位に集積を認めなかった.約5年後に肺転移が出現したため,S-1,GEM+5-FU+CDDP,GEM+CDDP+S-1を順に施行したが肺転移は緩徐に進行した.約10年後には皮膚転移,原発巣の増悪に伴うと思われる十二指腸出血を来し,初診から10年7ヶ月で癌死した.
症例は30代女性.上腹部痛を主訴に当院を受診.CT上,膵頭部に37mmの境界明瞭な充実性腫瘤を認め,内部は不均一でわずかに造影され,石灰化を有していた.EUS-FNAを施行し,膵芽腫と診断された.成人の膵芽腫はまれであるが,悪性度が高く,迅速なEUS-FNA診断が必須と考えられた.

肉芽腫性乳腺炎は稀な良性疾患で乳癌として手術され病理学的検査の結果肉芽腫性乳腺炎と診断される報告が散見される．今回われわれは，Corynebacterium tuberculostearicum（以下C. tuberculostearicum）感染が原因と思われる肉芽腫性乳腺炎の1例を経験したため報告する．症例は50代の女性．左乳房の疼痛を伴うしこりを主訴に受診した．触診上左C領域に40mm大の弾性硬・可動性良好な腫瘤を認めた．MMG・US・MRI上，乳癌が強く疑われ針生検を施行した．組織学的所見は，好中球中心の炎症細胞浸潤を認めたが悪性細胞は認めなかった．肉芽腫性乳腺炎を疑い腫瘤内容物を穿刺し培養に提出，抗菌薬投与を実施し，培養結果はC. tuberculostearicumであった．投与後4週間で腫瘤径の縮小を認め，診断後8カ月経過するが増悪なく経過している．
腹腔鏡手術は低侵襲であり整容性に優れるという特徴から広く普及している．腹部術後の癒着性イレウスは日常診療でしばしば遭遇する疾患であるが，イレウスの程度や患者の状態は症例ごとに大きく異なるため，腹腔鏡手術の適応は施設により異なる．イレウスの中には保存的加療により一時的に軽快するものの短期間に再発を繰り返す癒着性イレウスが存在する．癒着性イレウスでは癒着剝離のための再手術により再び癒着の原因を作ってしまうリスクが存在するが，腹腔鏡手術では創が小さいため再癒着のリスクを減らすことができる．当科ではこのような繰り返す癒着性イレウスで，十分減圧が得られた症例に対し，症状軽快後に待機的に腹腔鏡手術を施行し良好な成績を得ている．イレウスに対する腹腔鏡手術の報告は多数あるが比較的早期に手術が行われている報告が多く，待機的に行っているという報告は少ない．待機的に手術を施行することで炎症期に手術することを避け，開腹移行のリスクや患者の精神的負担を軽減するとこができると考える．今回，われわれは繰り返す癒着性イレウス３症例に対して待機的腹腔鏡下癒着剝離術を施行し経過が良好であったので報告する.
