1995～2001年に所沢肛門病院で経験した大腸癌症例の検討を行った.7年間に38,212例の大腸内視鏡検査が施行され,762例の大腸癌(進行癌498例,早期癌264例)が発見された.これは検査総数の約2.0%に達していた.進行癌では,男性の直腸癌が有意に多かった(p<0.01).平均年齢は進行癌(62.4歳)が早期癌(59.8歳)に比べ2.6歳高かった(p<0.002).当院の第一紹介病院である防衛医大第1外科で1995～99年に施行された大腸癌初回手術例743例を,当院から紹介した257例とその他の施設から紹介された476例に分け,両群の特徴を比較検討した.当院から紹介した症例では,主訴が出血である症例の割合が高く,腫瘍占拠部位では有意に直腸癌が多かった(p<0.0001).平均年齢は当院で有意に若年であるものの(p=0.02),臨床病期は有意に高く(p=0.01),痔疾患のある患者は出血しても痔からの出血と考え放置している可能性が示唆され注意が必要と考えられた.
