【目的】我々は頚部内頚動脈高度狭窄に起因する頭蓋内内頚動脈閉塞(tandem occlusion)を来した急性期脳梗塞症例を経験したため報告する.【症例】59 歳女性.右片麻痺と失語を主訴に発症から2 時間で救急搬送された.頭部MRIでclinical-diffusion-weighted image mismatch を認める左内頚動脈閉塞を認め,血管内治療を行った.頚部左内頚動脈高度狭窄の合併を認め,頚動脈ステント留置後に頭蓋内再開通療法を計画した.ステント留置後にpenumbra systemを使用し,発症より7 時間40 分で有効再開通が得られ,症状は改善した.【結論】頚部内頚動脈高度狭窄と頭蓋内内頚動脈閉塞症に対してcarotid artery stenting(CAS)とthrombectomy を行い,良好な治療経過を得た.頚動脈狭窄に起因した頭蓋内主要血管閉塞症に対してemergent CAS とthrombectomy は有用である.

前大脳動脈近位部 (A1) 動脈瘤は通常穿通枝分岐部より発生し, 開頭脳動脈瘤クリッピング術による治療難度の比較的高い病変である. 今回筆者が経験した6例について後方視的検討を加えた. 後方発育の割合が比較的多いこの部位の動脈瘤においては, 通常の脳血管撮影では動脈瘤が親血管に隠れ描出困難である場合が多く, その局在診断においては三次元回転血管造影法の有用性が示唆された. 開頭脳動脈瘤クリッピング術においては穿通枝の確実な同定と注意深いクリッピングにより穿通枝梗塞の合併回避が可能と考えられたが, 手術顕微鏡による形態学的評価のみでなく, 血流のリアルタイムイメージングであるインドシアニングリーン (ICG) を用いた術中蛍光血管撮影 (ICGA) が穿通枝温存および脳動脈瘤根治性の評価において本部位から発生した脳動脈瘤手術においては必須の検査であると考えられた. また, 術後のbrain shiftを念頭に置き, 動脈瘤頚部長に合ったブレード長のクリップの使用や有窓クリップの使用などが重要であることが示唆された.

症例は29歳男性である.2008年に肺サルコイドーシスを指摘された.2010年10月下旬に38°C台の発熱,頭痛が出現した.11月より膿疱性乾癬の診断で皮膚科へ入院したが,発熱,頭痛が改善せず当科に転科した.頭部MRIで脳底部を中心に微小造影結節をみとめた.脳脊髄液検査で細胞数,蛋白が増加し,糖は7 mg/dlと著明な低値を示した.抗結核薬,ステロイドパルス療法をおこなったが臨床症状,検査値ともに改善しなかった.髄膜生検で髄膜播種性サルコイドーシスと診断した.プレドニゾロン60 mg/日を開始し,症状は著明に改善した.髄膜播種性サルコイドーシスでも髄液糖が著明に低下する可能性に留意すべきである.
