個別の核酸増幅検査(NAT)で検出し得なかったB型肝炎ウイルス(HBV)を含む血液の輸血によるHBV感染症例を経験した.交通外傷による多臓器損傷が認められた20歳代男性に対し,赤血球濃厚液を24単位(12ドナー),新鮮凍結血漿を16単位(8ドナー),および,濃厚血小板を35単位(2ドナー)使用した.輸血後感染症検査でHBV-DNAが陽性であったため,輸血前保存検体の検査を行ったが,HBV関連マーカー(HBs抗原,HBs抗体,HBc抗体)はすべて陰性であった.供血した22ドナーの保管検体の個別NATもすべて陰性であったが,そのうちの1人は再度の献血で,NATが陽転化した.HBc抗体弱陽性でもあったため,occult HBV carrierとみなされた.輸血による感染の発端はこのドナー由来のFFPであった.現在の検査感度や日赤の輸血用血液の可否基準ではoccult carrierからの血液を確実に排除することは困難である.適正に輸血を行ってもウイルスに感染する可能性があるため,その感染と輸血との因果関係を証明するためには輸血前検体保存が必須である.
