<h3>Objective</h3> To estimate the size and characteristics of UK population with single-ventricle physiology, predict future growth. <h3>Methods</h3> The surviving physiology in Northern England (resident 2.9 million) was identified from our clinical database Congenital Abnormality Survey (NorCAS). Conditions included double inlet ventricle, tricuspid atresia, mitral hypoplastic left heart syndrome other unbalanced defects. Fetal diagnoses, terminations pregnancy surgical interventions were...
Objectives Adults with congenital heart disease (ACHD) are a growing group end-stage failure. We aim to describe the outcomes of ACHD patients undergoing assessment for orthotopic transplant (OHT). Methods Case notes consecutive (>16 years) assessed OHT between 2000 and 2016 at our centre were reviewed. Decision outcome reported as 2017. Data analysed in three groups: systemic left ventricle (LV), right (RV) single (SV). Results 196 (31.8 years, 27% LV, 29% RV, 44% SV). 89 (45%) listed 67...
Abstract Objective : It is accepted that changes in the peripheral pulse waveform characteristics occur with ageing. Pulse risetime one important feature which has clinical value. However, it unclear how varies across full age spectrum from child to senior and for different measurement sites. The objectives of this study were determine association between over an 8-decade range at ears, fingers, toes, consider effects arising differences systolic blood pressure (SBP), height heart rate....
With the scarcity of organs, a durable, reliable ventricular assist device (VAD) is required. The Berlin Heart EXCOR ® (BH) remains most established VAD in paediatric population. Implantable continuous flow (CF) VADs have been introduced to field with encouraging early results. In this study, we compared results newly CF (HeartWare [HVAD] ) matched group BH recipients. The study included patients aged <16 years who received mechanical left (LVAD) support between December 2005 and January...
Objective: It has been documented that heart transplantation in children is often complicated by arterial hypertension and increased stiffness. We use innovative multi-site photoplethysmography (MPPG) pulse measurement analysis technology to assess changes stiffness paediatric transplant recipients (HTRs) comparison with healthy control (HC) children. Approach: A group of 20 HTRs (median age 13.5 years, eight male) were compared an overall age- gender-matched 161 HCs 11.6 74 male)....
The onset of pulmonary hypertension in patients with systemic sclerosis carries a poor prognosis. Atrial septostomy has been used successfully to palliate endstage primary but not attempted other forms vascular disease. We report substantial clinical improvement following atrial patient complicated by severe, isolated hypertension. After the procedure, exercise capacity was improved and exertional syncope abolished. suggest that this procedure should be considered for diagnosis.
Abstract Management of adults with failing Fontan physiology poses many challenges, especially as transplantation offers the only realistic alternative to palliative care. We present first combined heart and liver transplant performed in Europe, for a late survivor single ventricle palliation circulation. In addition conventional medical surgical challenges posed, we highlight management associated multi‐organ failure focus on novel strategies assessment optimization.
Our goal was to present 2 decades of our experience with the Ross procedure and its sequential modifications, adopted since 2010, improve reoperation rate.We performed a single-centre, retrospective review database information medical notes about implantation technique: freestanding root. We compared era 1 (1997-2009) (2010-2019).Between 1997 2019, total 214 procedures were (71% men, median age 24 years) [interquartile range (IQR) 15-38]. Of these, 87% had various forms congenital-dysplastic...
Ventricular assist device (VAD) for systemic right ventricular (RV) failure patients post-atrial switch, transposition of the great arteries (TGA), and those with congenitally corrected TGA has proven useful to reduce transpulmonary gradient bridge-to-transplantation. The purpose this study is describe our experience VAD in RV move towards concomitant tricuspid valve replacement (TVR).This a single-centre retrospective consecutive adult receiving HeartWare between 2010 2019. From 2017, TVR...
Chronic anemia is common in adults after successful cardiac transplantation. However, the prevalence of children transplantation uncertain. The purpose this study was to investigate and causes chronic well particular define role, if any, iron deficiency, which important relatively normal children.Twenty (ages 7 months 16 years) who were 4 6 years studied. Fourteen (70%) anemic enrolled a prospective trial supplementation. In majority children, serum erythropoietin levels low, although...
The Fontan operation greatly improves survival for single ventricle congenital heart disease patients but creates a physiology that leads to long-term multi-organ dysfunction. A non-invasive screening tool can identify impending decline is sought. objective of this pilot study was assess the microcirculation in Fontan-palliated by measuring tissue oxygen saturation (StO2) superficial and deeper tissues.Three patient cohorts were studied: group (n = 8) two control groups, liver tetralogy...
Background Patent foramen ovale (PFO) occurs in a quarter of the population but half those with cryptogenic stroke (CS). Difficulty identifying pathogenic PFO versus innocent bystander has contributed to controversy surrounding outcomes following closure. We aimed investigate whether right atrial flow patterns could help define mechanism for CS setting PFO. Methods 4D cardiac magnetic resonance (CMR) was performed 10 subjects (age 43±6 years, 4 male) proven and controls 42±7years, normal...
Most reports on the outcome of children who present with heart failure, due to muscle disease, are from an era when ventricular assist devices were not available. This study provides data for current where prolonged circulatory support can be considered most children.Data was retrieved 100 consecutive children, presented between 2010 - 2016, a first diagnosis unexplained failure. Hospital classified as either death, transplantation, recovery function or persistent Median age at presentation...
Abstract Background One of the most frequent complications left ventricular assist device (LVAD) implantation is development right (RV) failure, which occurs in 10%-40% patients. It is, therefore, essential to identify LVAD candidates are at risk for RV failure and will require additional mechanical support with a biventricular (BiVAD). However, echocardiographic assessment challenging due its complex geometry marked load dependence function indices. To our knowledge, there no published...
A PC based vibration monitoring system can be an effective tool for the prevention and diagnosis of related problems in microelectronics facilities. The computing storage power present day PC''s, coupled with availability high speed data acquisition plug-in boards, allows creation a powerful but low cost system. frequency content motion typical buildings that support sensitive tools exploited by multiplexing multiple signals into single A/D converter. By doing that, many channels monitored...
