胸腺腫肝転移の画像検査の特徴的所見および外科的切除の有効性に関する詳細な報告は少ない．症例は79歳，男性．肝腫瘍の精査加療目的に当院を紹介受診した．充実成分と嚢胞成分を有する肝腫瘍に対して肝切除術を行った．病理組織学的検査で胸腺腫肝転移と診断されたため，肝切除術5カ月後に前縦隔腫瘍摘出術および転移性肺腫瘍切除術を施行，病理組織学的検査は胸腺腫および胸腺腫肺転移であった．術後補助化学療法は施行していないが，術後42カ月無再発生存中である．胸腺腫の血行性転移は頻度が低く，画像検査所見の特徴は明らかではない．肝臓の嚢胞性病変は様々な鑑別疾患が挙げられるが，胸腺腫肝転移も嚢胞成分を伴っていることがある．胸腺腫肝転移に対する外科的切除が有用である症例が存在することが本症例から示唆された．
Body mass index（以下，BMIと略記）35kg/m2以上の高度肥満を伴う大腸癌に対し腹腔鏡下手術を施行した4例を検討した．数値を中央値（範囲）で示す．年齢64（54-80）歳，男性2例，女性2例，BMI 41.6kg/m2（35.6-48.7）．2例において術前のエネルギー制限を行った．術式は回盲部切除術，低位前方切除術，S状結腸切除術，結腸左半切除術それぞれ1例．手術時間393（335-688）分，術中出血量173.5（40-1,790）mL，術後在院日数10（7-63）日．1例で手術創の追加，2例でポートの追加を行った．低位前方切除術の症例において出血制御不能のため開腹移行し，術後に縫合不全を認めた．高度肥満を伴う大腸癌症例に対する腹腔鏡手術において，手術操作上の困難性を克服することが重要と考えられた．また，術前減量の意義については症例集積のうえ今後も検討を要する．
当院で経験した肛門管扁平上皮癌6例に対する化学放射線療法の治療成績を後方視的に検討した．臨床病期はIが1例，IIIAが1例，IIIBが1例，IIICが3例であった．初回治療として1例に局所切除が施行された．併用化学療法はfluorouracil+mitomycin Cが4例，fluorouracil+cisplatinが2例であった．Grade3以上の有害事象を3例に認め，内訳は好中球減少1例，皮膚炎1例，嘔気1例であった．自然肛門は4例で温存可能であった．他2例は腫瘍による腸閉塞のため，化学放射線療法開始前に人工肛門を必要とした1例と，化学放射線療法後肛門機能廃絶のため人工肛門造設術を施行した1例であった．治療成績は全例で完全奏効が得られ，無再発生存中である．肛門管扁平上皮癌に対する化学放射線療法は，有害事象に注意を要するが根治が期待される有用な治療法と考えられた．
症例は60歳代，男性．腹痛のため前医を受診し，S状結腸憩室穿孔と診断され当院へ紹介された．HincheyIIIのS状結腸憩室炎・穿孔と診断し，腹腔鏡下ハルトマン手術を施行した．術後合併症なく経過し，初回手術から4ヵ月後に腹腔鏡下ハルトマンリバーサル手術を行った．人工肛門を直視下に閉鎖した後に，腹腔鏡下に直腸断端の癒着を剥離した．直腸を授動した後に，結腸と直腸を端側吻合した（single stapling technique）．術後経過は良好で，合併症なく退院した．S状結腸憩室穿孔に対するハルトマン手術，またその消化管再建術であるハルトマンリバーサル手術を腹腔鏡下に行うことの有用性が報告されているものの，本邦での報告は少ない．文献的な考察を含め報告する．
