Acid suppression with famotidine, a histamine H2–receptor antagonist, provides protection against gastric injury in normal subjects receiving short courses of aspirin or naproxen. The efficacy famotidine preventing peptic ulcers patients long-term therapy nonsteroidal antiinflammatory drugs (NSAIDs) is not known.
Previous studies have failed to identify mutations in the Wilson's disease gene ATP7B a significant number of clinically diagnosed cases. This has led concerns about genetic heterogeneity for this condition but also suggested presence unusual mutational mechanisms. We now present our findings 181 patients from United Kingdom with and biochemically confirmed disease. A total 116 different were detected, 32 which are novel. The overall mutation detection frequency was 98%. likelihood genes...
Oesophageal adenocarcinoma is the sixth most common cause of cancer death worldwide and Barrett's oesophagus biggest risk factor. We aimed to evaluate efficacy high-dose esomeprazole proton-pump inhibitor (PPI) aspirin for improving outcomes in patients with oesophagus.
A well defined cohort of coeliac patients was studied prospectively to assess the prevalence coexisting thyroid disease and positive autoantibodies. Comparison with epidemiological data on in a neighbouring area suggested that few adult had been missed. Overall, 14% disease: 10.3% were hypothyroid 3.7% hyperthyroid, both significantly more than expected. There autoantibodies expected--11% thyroglobulin antibodies 15% microsomal antibodies. This association is clinically important. Three are...
OBJECTIVES: We performed two 24-week double-blind trials (REDUCE-1 and -2 (Registration Endoscopic Studies to Determine Ulcer Formation of HZT-501 Compared with Ibuprofen: Efficacy Safety Studies)) assess whether double-dose famotidine given in a single-tablet combination ibuprofen (HZT-501) significantly reduces gastric duodenal ulcers as compared ibuprofen. METHODS: Patients (40–80 years) requiring daily non-steroidal anti-inflammatory drugs (NSAIDs) for ≥6 months no prior ulcer...
AIMS: To evaluate the prevalence and significance of chemical gastritis, in comparison with gastritis related to Helicobacter pylori patients receiving non-steroidal anti inflammatory drugs (NSAIDs). METHODS: Two hundred eighteen were studied, 174 whom taking NSAIDs. Chemical was defined as presence foveolar hyperplasia, muscle fibres lamina propria, oedema vasodilation, absence a chronic cell infiltrate. RESULTS: found 46 (26%) NSAIDs, three (7%) subjects not these (p less than 0.01). H...
The importance of erosions in longterm non-steroidal anti-inflammatory drug (NSAID) users, their relevance to ulceration and relation Helicobacter pylori are unclear. This study assessed the incidence peptic ulcers presence or absence H a group NSAID users (n = 50), undergoing endoscopy at 0, 4, 12, 24 weeks while continuing with NSAIDs. Ulcers diagnosed baseline were excluded. developed nine 23 patients (39%) pre-existing compared six 27 (22%) without (p < 0.05). infected 30) had total 18...
Duodenitis and gastric metaplasia, which is often colonised by Helicobacter pylori (H pylori), are increasingly recognised for their importance in the pathogenesis of duodenal ulcers. The situation not clear patients receiving non-steroidal anti-inflammatory drugs (NSAIDs), who have a higher risk peptic ulceration. aim this study was to identify histological abnormalities presence or absence NSAIDs, H pylori, Endoscopic biopsy specimens were taken from healthy looking mucosa 172 (74 took 98...
Summary Background: Low‐dose aspirin and other anti‐thrombotic therapy has been increasingly used for vascular protection. Aim: To assess the possibility that incidence of upper gastrointestinal blood loss changed in subjects using these agents comparison with non‐steroidal anti‐inflammatory drugs. Methods: We studied characteristics all patients acute haemorrhage attending a single hospital at 3 points over 6‐year period: 1996 ( n = 204), 1999 224) 2002 252). Results: The taking low‐dose...
The host9s humoral immune response to Helicobacter pylori has been used in the diagnosis of active infection with these organisms. Several commercial tests are available but there few and unconfirmed reports their efficacy. This study aimed assess compare efficacy following H serological patients treated or not non-steroidal anti-inflammatory drugs (NSAID): Pyloriset Latex, Helico-G, Biolab Malakit, Bio-Rad GAP Test IgG. Venous blood was tested at random 124 patients, 64 whom had received...
SUMMARY Using laser Doppler flowmetry, we measured gastric and duodenal mucosal blood flow in 70 patients who had taken non‐steroidal anti‐inflammatory drugs (NSAIDs) for longer than 4 weeks, studied the correlation with demographic factors, ulceration, Helicobacter pylori . Blood was also 17 other subjects not taking any drugs. Measurements were from healthy‐looking mucosa antrum first part of duodenum. Both values significantly lower NSAID those did not. In group, median 150 perfusion...
The synthesis of gastric and duodenal mucosal prostaglandin E2, I2, thromboxane B2 during a 60 minute incubation biopsy specimens, the degree endoscopic histological damage, anti-inflammatory response were all studied after four week, double blind study therapeutic doses two non-steroidal drugs, naproxen etodolac, received by 27 patients with active rheumatoid arthritis (13 receiving naproxen, 14 etodolac). Prostaglandin values treatment did not differ from baseline levels when analysed as...
Summary Background It is not clear whether the incidence or early mortality related to peptic ulcer perforation has changed. Aim To evaluate and while considering intake of low‐dose aspirin nonsteroidal anti‐inflammatory drugs (NSAIDs). Methods We recorded numbers details all patients presenting in our region Scotland with between 1997 2006 including demography, drug usage 30‐day mortality. Results In subjects aged >65 years, annual was 32.7 per 10 5 age‐specific population, whom 10.7...
Summary Background Little is known about the site and nature of bleeding lesions related to low‐dose aspirin other antithrombotic agents. Aim To describe mucosal abnormalities in patients presenting with upper gastrointestinal while being treated these drugs. Methods The endoscopic findings clinical details were analysed all haematemesis and/or melaena at a single centre during three calendar years. Associations between risk factors, including intake non‐steroidal anti‐inflammatory drugs,...
Summary Background Rofecoxib was withdrawn in 2004. Aim To assess the incidence of upper gastrointestinal bleeding context changing use cyclo‐oxygenase‐2 non‐steroidal anti‐inflammatory drugs and low‐dose aspirin. Methods We examined characteristics patients developing a defined population south‐west Scotland. The primary comparisons were made between two calendar years, preceding following withdrawal rofecoxib. Results overall rose from 98.7 2002 to 143 per 10 5 annum 2005 ( χ 2 = 21.1; P...
Although ulcers are often associated with non-steroidal anti-inflammatory drugs (NSAIDs) little is known about the feasibility of predicting their development in patients taking NSAIDs. In addition, ulcerogenic potentials newer NSAIDs, taken on long term basis, have not been compared those more established preparations. The aim this study was to identify clinical and pathological characteristics at a higher risk NSAID induced ulcers, measure potential variety test effect these predictability...
