Upper airway injury is a recognized complication of prolonged endotracheal intubation, yet little attention has been paid to the consequences laryngeal and functional impact. The purpose our study was prospectively define incidence acute investigate impact on breathing voice outcomes.Prospective cohort study.Tertiary referral critical care center.Consecutive adult patients intubated greater than 12 hours in medical ICU from August 2017 May 2018 who underwent laryngoscopy within 36...
Background The incidence of oral tongue squamous cell carcinoma (OTSCC) is increasing among younger birth cohorts. etiology early‐onset OTSCC (diagnosed before the age 50 years) and cancer driver genes remain largely unknown. Methods Sequencing Consortium Oral Tongue Cancer was established through pooling somatic mutation data specimens (n = 227 [107 cases]) from 7 studies Genome Atlas. Somatic mutations at microsatellite loci Catalog Mutations in signatures were identified. identified with...
Abstract Background Incidence of oral tongue squamous cell carcinoma (OTC) is rising among those under age 50 years. The etiology unknown. Methods A total 395 cases OTC diagnosed and/or treated at Vanderbilt University Medical Center between 2000 and 2017 were identified. Of those, 113 (28.6%) early onset (age < years). Logistic regression was used to identify factors associated with OTC. Cox proportional hazards models evaluated survival recurrence. Results Compared typical patients,...
Endotracheal intubation and mechanical ventilation are life-saving treatments for acute respiratory failure but complicated by significant rates of dyspnea dysphonia after extubation. Unilateral vocal fold immobility (UVFI) extubation can alter respiration phonation, its incidence, risk factors, pathophysiology remain unclear. To determine the incidence UVFI prolonged (>12 hours) in a medical intensive care unit investigate associated clinical factors ventilation. This subgroup analysis...
To measure disease-free, disease-specific, and overall survival among patients with T4aN0M0 mandibular gingival squamous cell carcinoma who were treated surgery alone.Case series chart review.Tertiary care center.A retrospective review was performed of all adult surgically an oral cavity composite resection between January 2005 March 2017. Among other data, patient preoperative characteristics recorded (eg, age, sex, smoking history, alcohol use, clinical stage); operative notes reviewed to...
Many patients who survive critical illness are left with laryngeal functional impairment from endotracheal intubation that permanently limits their recovery and quality of life. Although the risk for injury increases larger tube sizes, there no data delineating association smaller sizes survival or acute illness. To determine if tubes noninferior to respect outcomes. This propensity score-matched retrospective cohort study included all adult underwent in emergency department intensive care...
The social determinants of health affect a wide range outcomes and risks. To date, there have been no studies evaluating the impact on laryngotracheal stenosis (LTS). We sought to describe in cohort LTS patients explore their association with treatment outcome.Subjects diagnosed undergoing surgical procedures between 2013 2018 were identified. Matched controls identified from intensive care unit (ICU) who underwent intubation for greater than 24 hours. Medical comorbidities, characteristics,...
Preclinical studies suggest learned immune system responses to alcohol cues and consumption may contribute alcohol's pharmacodynamic properties and/or Alcohol Use Disorder (AUD) pathogenesis. Mechanistically, these alterations be associated with increased craving consumption, both acutely over time. We sought characterize this relationship in a randomized, counter-balanced, crossover neuroimaging experiment which took place between June 2020-November 2021.
Dermoid tumors are benign inclusion cysts that uncommon in the pediatric population. of middle ear exceedingly rare. A 17-month old female presented with 4-month history recurrent left otitis media chronic drainage, speech delay, and worsening episodes stumbling while walking. temporal bone CT scan showed complete mastoid air cell opacification. The patient underwent tympanomastoidectomy for removal mass pathologic confirmation a dermoid tumor. continues to do well post-operatively without...
(1) Background/Objectives: Delayed esophageal perforation following anterior cervical (spine) discectomy and fusion (ACDF) is rare but can lead to serious infectious complications. The treatment usually involves hardware explanation prolonged intravenous antibiotics; however, there are scarce reports about the microbiology of these infections corresponding targeted therapy. (2) Methods: Patients diagnosed or treated for delayed after between 2000-2020 at a tertiary medical center were...
Abstract The purpose of this study was to evaluate the use ultrasound in identifying fetal position and cervical spine flexion breech presentations. Radiographic ultra‐sonographic determinations type presentation (frank, complete or incomplete) degree 52 presentations were compared. angle defined as between mandible main axis categorized full flexion, military, partial extension extension. Ultrasound concordant with radio‐graphic assessment 77% 64% subjects even more accurate those labor....
We present the case of a patient who developed an isolated palsy flexor pollicis longus (FPL) branch anterior interosseous nerve (AIN) following fracture right radial shaft. The diagnosis AIN in this setting is rare, especially involving partial neuropathies only FPL branch. Clinical presentation scenario can be mistaken for other musculoskeletal pathology, and electrodiagnostic studies helpful confirming diagnosis.
