症例は87歳の女性で, 11年前に子宮頚癌に対して50Gyの放射線治療を受けた既往があった.呼吸困難, 倦怠感などの自覚症状および胸部画像所見が急速に進展し, 入院後5日目に急性呼吸不全で死亡した.画像上は境界不鮮明な多発性小結節影に加え, スリガラス様陰影や浸潤影などの肺胞領域病変と小葉間隔壁の肥厚など間質病変を思わせる陰影が混在していた.剖検では, 子宮に6.0×4.5cmの血腫様腫瘤が認められ, 同様の腫瘤が肺, 肝, 脾にも多発性に認められ, 子宮血管肉腫, およびその肺, 脾転移と診断された.子宮への放射線照射の既往があることから, 子宮血管肉腫は二次癌と考えられた.胸部画像上, 小結節の境界が不鮮明で, スリガラス様陰影を伴っていたのは転移性腫瘍と腫瘍周囲の肺出血によるものであった.子宮原発の血管肉腫や転移性肺血管肉腫は, 非常に稀であり, 文献的考察を加え報告する.
