The purpose of this study was to evaluate three-dimensional images liver tumours obtained with gadolinium ethoxybenzyl diethylenetriamine penta-acetic acid (Gd-EOB-DTPA)-enhanced MRI (3D-EOB-MRI) in hepatic surgery. We conclude that 3D-EOB-MRI may be an alternative method for depicting adjacent the veins and portal branches, provide additional information surgical planning.
症例は21歳の男性. ウイルス性咽頭炎の診断で入院加療となったが, 入院後全身状態が悪化し, 骨髄穿刺検査所見により血球貪食症候群と診断された. 患者が同性愛者であったことからHIV抗体を測定したが, 陰性であり, その後HIV-1 RNA定量よりHIVウイルス感染症と判明し, HIV初感染によるVAHSと診断した.本症例の経験から, ウイルス性咽喉頭炎様の症状が長引く時には急性HIV症候群も考慮し, HIV抗体のみならず, HIV-RNA量を測定する必要があると考えられた.

頭頸部進行癌に超選択的動注療法が施行され，高い局所制御と遠隔転移の問題が指摘されている。目的：Cimetidine併用による遠隔転移抑制の有無を検討。対象は2000年5月より2009年12月に動注療法を施行した切除不能153例。Cimetidine投与群114例は内服希望例，非投与群39例は内服拒否例である。方法：治療前よりCimetidineを1年内服の投与群と非投与群で遠隔転移を比較検討した。動注療法を1週毎に4回施行した。治療2ヶ月後PETで局所効果と遠隔転移を判定，以後血液検査2ヶ月，胸部CT 6ヶ月，胸部写真を3ヶ月毎に実施した。結果：観察期間の中央値は投与群45ヶ月，非投与群64ヶ月（p < 0.05）。非投与群に遠隔転移10例と投与群に遠隔転移6例を認め，有意に抑制された。平均転移時期は6.9ヶ月（2-20）であった。転移部位は肺8例，骨5例，脳2例，腹部1例であった。原発巣は副鼻腔5例，下咽頭4例，口腔5例，その他2例であった。結論：局所治療の動注療法に遠隔転移抑制のCimetidineの併用は有用と考えた。
We investigated the effectiveness of Lipo-prostaglandin E1 (Lipo-PGE1) into patients with idiopathic sudden hearing loss. Paragangliomas are highly vascular, slow growing neoplasms that can occur within the temporal bone, as well other sites in head and neck.
