症例1は78歳，男性．心窩部痛を主訴に来院．右下腹部に筋性防御，採血でCEAが高値．腹部X線，腹部CT所見で腹腔内遊離ガス像はないが，腸間膜内に異常なガス像があり，癌合併腹膜炎として緊急手術施行．上行結腸癌と腸間膜内に糞便を認め，右半結腸切除を施行．標本は上行結腸に1型の腫瘍と，その口側の腸間膜側に穿通があり，その粘膜には虚血性変化を認めた．症例2は73歳，男性．腹痛にて当院に救急搬送される．腹部CT検査で腸間膜内に異常ガス像を認め，上行結腸間膜穿通と診断，緊急手術施行．腸間膜内に多量の便塊と上行結腸癌を認め，右半結腸切除を施行．標本は上行結腸に3型の腫瘍と口側の腸間膜側に穿通部を認めた．病理では穿通部に循環障害を指摘された．いずれも比較的順調に経過し，退院した．本症は稀な腸間膜穿通を合併した上行結腸癌だが，診断には腹部CT検査が有用であり，緊急手術で一期的吻合を行い救命しえた．
