当院で経験した痔瘻癌15例の臨床病理学的特徴と治療成績の検討を行った.男性12例,女性3例,平均年齢59歳,痔瘻から癌発見までの期間は中央値19年.組織型は粘液癌が67%で,浸潤臓器は肛門挙筋,尾骨,精嚢前立腺,仙骨に及んだ.14例に腹会陰式直腸切断術(うち2例は腹腔鏡下手術)を,1例に局所切除術を施行し,2例に筋皮弁による再建を併施した.組織学的リンパ節転移はpN0:12例,pN2:1例,pN3:1例で,鼡径リンパ節転移は認めなかった.病期はfStage I:4例,fStage II:9例,fStage IIIB:2例であり,病理学的剥離面癌露出の2例は,いずれも局所再発した.全15例における5年生存率は69%であった.再発例でも集学的治療が奏効し長期生存する症例を経験した.本報告を含め,痔瘻癌本邦報告例119例の検討も加え,報告する.
