Fontan術後の蛋白漏出性胃腸症(PLE)に対しトルバプタンを使用してPLEが改善した2症例を報告する.症例1は無脾症候群,右胸心,共通房室弁口,単心室,肺動脈閉鎖,総肺動脈還流異常症(TAPVC)のFontan術後の8歳男児.アルブミン補充,ステロイド投与にもかかわらずAlb2g/dL台,TP 3g/dL台と低値だったが,トルバプタン投与後より正常範囲内となりステロイドを漸減できた.またPLEによる下痢も改善し普通便となった.症例2は多脾症候群,単心室,共通房室弁口,両大血管右室起始,肺動脈閉鎖,下大静脈欠損(奇静脈交通)に対するFontan術後の19歳男性.トルバプタン投与前はAlb 2.0g/dL,TP 4.0g/dLだったがトルバプタン投与により正常範囲内に回復し,腹水も軽減した.トルバプタンがFontan術後PLEに対する内科的治療の一選択肢となりうる可能性が示唆された.
