Oral cancers rarely metastasize to the small intestines. In a previously operated case of squamous cell carcinoma buccal mucosa without any known preoperative distant metastases, we report solitary ileal perforation 3 months after surgery. The edge was positive for on histopathology. It is important remember metastases as cause acute abdomen in prior history oral malignancies.
