症例は66歳女性である.脳膿瘍で排膿術施行され,以後はメトロニダゾール2 g/日を投与された.30日目より嘔気出現し徐々に悪化,45日目に昏迷になった.意識変容,軽度の前庭障害と構音障害をみとめた.頭部MRIは脳梁膨大部,小脳歯状核,脳幹部下丘にDWIおよびFLAIRで高信号変化を両側対称性にみとめた.脳梁膨大部のapparent diffusion coefficient(ADC)は低下,MRSで乳酸のピーク上昇をみとめた.メトロニダゾール誘発性脳症と診断し,同薬剤を中止し,症状は改善し14日目に軽快退院した.小脳歯状核および脳幹病変は,血管障害性浮腫と思われ5~10日目には消失したが,脳梁膨大部病変は細胞毒性浮腫と思われ40日目まで残存した.

硬膜下血腫患者連続1,009症例を対象とし,痙攣を併発した26例(2.6%)のうち血腫が痙攣を誘発した可能性のある20例をケース(痙攣群)として,性,年齢,手術の有無を1:2にマッチさせたコントロール群(非痙攣群)40例と比較した.痙攣群で認知症が多かったが,その他臨床項目で差異はみとめなかった.痙攣群で両側性血腫が多い傾向であり,MRIのFLAIR画像における脳溝の高信号(sulcal hyperintensity)を呈する症例が有意に多く,この所見を呈する症例はCT画像では混合性血腫内容を呈するものが多かった.血腫内容が脳実質へ浸潤して,刺激成分が痙攣を誘発する機序が推測された.
