BACKGROUND. SARS-CoV-2 plasma viremia has been associated with severe disease and death in COVID-19 small-scale cohort studies. The mechanisms behind this association remain elusive.
We enrolled 7 individuals with recurrent symptoms or antigen test conversion following nirmatrelvir-ritonavir treatment. High viral loads (median 6.1 log10 copies/mL) were detected after rebound for a median of 17 days initial diagnosis. Three had culturable virus up to 16 No known resistance-associated mutations identified.
Despite vaccination and antiviral therapies, immunocompromised individuals are at risk for prolonged severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, but the immune defects that predispose an individual to persistent disease 2019 (COVID-19) remain incompletely understood. In this study, we performed detailed viro-immunologic analyses of a prospective cohort participants with COVID-19. The median times nasal viral RNA culture clearance in immunosuppression due...
The orthogonal sulfur-fluoride exchange reaction (SuFEx) and copper(I)-catalyzed azide-alkyne cycloaddition (CuAAC) are employed to synthesize sequence-regulated synthetic polymers. high efficiency broad tolerance of SuFEx CuAAC diverse chemical functionalities enable the one-pot synthesis polydispersed sequence-controlled polymers by step-growth copolymerization in yield sequence complexity. Furthermore, iterative coupling reactions on a solid support, without need protecting groups, afford...
There is limited information on the specific impact of maternal infection with SARS-CoV-2 B.1.617.2 (delta) variant pregnancy outcomes. We present 2 cases intrauterine fetal demise and 1 case severe distress in setting delta-variant SARS-CoV-2. In all cases, or occurred within 14 days COVID-19 diagnosis. Evaluation revealed viremia, high nasopharyngeal viral load, evidence placental SARS-CoV-2, hallmark features placentitis. suggest that during warrants vigilance for dysfunction compromise...
The effects of high-dose inhaled nitric oxide on hypoxemia in coronavirus disease (COVID-19) acute respiratory failure are unknown.
Isolation guidelines for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) are largely derived from data collected prior to the emergence of delta variant. We followed a cohort ambulatory patients with postvaccination breakthrough SARS-CoV-2 infections longitudinal collection nasal swabs viral load quantification, whole-genome sequencing, and culture. All variant in our were symptomatic, compared 64% non-delta infections. Symptomatic characterized by higher initial load, longer...
Clinical features of SARS-CoV-2 Omicron variant infection, including incubation period and transmission rates, distinguish this from preceding variants. However, whether the duration shedding viable virus differs between omicron previous variants is not well understood. To characterize how vaccination status impact virus, we serially sampled symptomatic outpatients newly diagnosed with COVID-19. Anterior nasal swabs were tested for viral load, sequencing, culture. Time to PCR conversion was...
Abstract O‐sulfation is an important chemical code widely existing in bioactive molecules, but the scalable and facile synthesis of complex molecules carrying O‐sulfates remains challenging. Reported here a general approach to by sulfur(VI) fluoride exchange (SuFEx) reaction between aryl fluorosulfates silylated hydroxy groups. Efficient sulfate diester formation was achieved through systematic optimization electronic properties fluorosulfates. The versatility this strategy demonstrated...
Protective immunity against SARS-CoV-2 infection after COVID-19 vaccination may differ by variant. We enrolled vaccinated (n = 39) and unvaccinated 11) individuals with acute, symptomatic Delta or Omicron performed viral load quantification, whole-genome sequencing, variant-specific antibody characterization at the time of acute illness convalescence. Viral was inversely correlated binding neutralizing responses. Across all variants tested, convalescent neutralization titers in were markedly...
We assessed the ability of BinaxNow rapid test to detect severe acute respiratory syndrome coronavirus 2 antigen from 4 individuals with Omicron and Delta infections. performed serial dilutions nasal swab samples, specimens concentrations ≥100 000 copies/swab were positive, demonstrating that is able variant.

Abstract The impact of coronavirus disease 2019 vaccination on viral characteristics breakthrough infections is unknown. In this prospective cohort study, incidence severe acute respiratory syndrome 2 infection decreased following vaccination. Although asymptomatic positive tests were observed vaccination, the higher cycle thresholds, repeat negative tests, and inability to culture virus raise questions about their clinical significance.
Abstract There is increasing evidence that the risk of SARS-CoV-2 infection among vaccinated individuals variant-specific, suggesting protective immunity against may differ by variant. We enrolled (n = 39) and unvaccinated 11) with acute, symptomatic Delta or Omicron performed viral load quantification, whole-genome sequencing, variant-specific antibody characterization at time acute illness convalescence. Viral was inversely correlated binding neutralizing responses. Increases in titers...
Persistent SARS-CoV-2 replication and systemic dissemination are linked to increased COVID-19 disease severity mortality. However, the precise immune profiles that track with enhanced viral clearance, particularly from RNAemia, remain incompletely defined. To define whether antibody characteristics, specificities, or functions emerge during natural infection accelerated containment of replication, we examined relationship SARS-CoV-2-specific humoral evolution in setting plasma which is...

Resistance mutations to monoclonal antibody (mAb) therapy has been reported, but in the non-immunosuppressed population, it is unclear if vivo emergence of SARS-CoV-2 resistance alters either viral replication dynamics or therapeutic efficacy. In ACTIV-2/A5401, non-hospitalized participants with symptomatic infection were randomized bamlanivimab (700mg 7000mg) placebo. Treatment-emergent significantly more likely detected after 700mg treatment than placebo (7% 111 vs 0% 112 participants,...

Abstract Background Monoclonal antibodies (mAbs) represent a crucial antiviral strategy for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, but it is unclear whether combination mAbs offer benefit over single-active mAb treatment. Amubarvimab and romlusevimab significantly reduced the risk of hospitalizations or death in ACTIV-2/A5401 trial. Certain SARS-CoV-2 variants are intrinsically resistant against romlusevimab, leading to only therapy with amubarvimab these...
Abstract Widespread, frequent testing is essential for curbing the ongoing COVID-19 pandemic. Because its simplicity makes it ideal widely distributed, high throughput testing, RT-LAMP provides an attractive alternative to RT-qPCR. However, most protocols require purification of RNA, a complex and low-throughput bottleneck that has often been subject reagent supply shortages. Here, we report optimized RT-LAMP-based SARS-CoV-2 diagnostic protocol saliva swab samples. In replace RNA with...
Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) kinetics remain understudied, including the impact of remdesivir. In hospitalized individuals, peak sputum viral load occurred in week symptoms, whereas viremia peaked within 1 symptom-onset, suggesting early systemic seeding SARS-CoV-2. Remdesivir treatment was associated with faster decay.
