The ItalKid Project is a prospective, population-based registry that was started in 1990 with the aim of assessing epidemiology childhood chronic renal failure (CRF), describing natural history disease, and identifying factors influence its course. This article reports epidemiologic results.Prevalent incident cases CRF children adolescents were identified throughout Italy (total population base: 16.8 million children) by regularly asking all pediatric hospitals adult nephrology units...

Prednisone and calcineurin inhibitors are the mainstay therapy of idiopathic nephrotic syndrome (INS) in children. However, drug dependence toxicity associated with protracted use common. Case series suggest that anti-CD20 monoclonal antibody rituximab (RTX) may maintain disease remission. This open-label randomized controlled trial was powered to show a strategy based on RTX lower doses prednisone noninferior standard these agents maintaining 3-month proteinuria as low baseline or up 1 g/d...

Abstract The mean annual incidence of hemolytic uremic syndrome in persons <15 years age Italy from 1988 to 2000 was 0.28 per 100,000 population. Laboratory investigations showed that Shiga toxin–producing Escherichia coli (STEC) infection occurred 73.1% patients. STEC O157 the most common serotype, but a considerable number cases were infections by non-O157 STEC.

Steroid-dependent nephrotic syndrome (SDNS) carries a high risk of toxicity from steroids or steroid-sparing agents. This open-label, noninferiority, randomized controlled trial at four sites in Italy tested whether rituximab is noninferior to maintaining remission juvenile SDNS. We enrolled children age 1-16 years who had developed SDNS the previous 6-12 months and were maintained with prednisone doses (≥0.7 mg/kg per day). randomly assigned participants continue alone for 1 month (control)...

Idiopathic nephrotic syndrome resistant to standard treatments remains a therapeutic dilemma in pediatric nephrology. To test whether the anti-CD20 monoclonal antibody rituximab may benefit these patients, we conducted an open-label, randomized, controlled trial 31 children with idiopathic unresponsive combination of calcineurin inhibitors and prednisone. All continued prednisone at doses prescribed before enrollment, one treatment group received two (375 mg/m(2) intravenously) as add-on...

Very young children with chronic kidney disease often have difficulty maintaining adequate nutrition, which contributes to the high prevalence of short stature in this population. Characteristics dialysis prescription and supplemental feeding via a nasogastric (NG) tube or gastrostomy may improve growth, but is not well understood. Here, we analyzed data from 153 18 countries who commenced peritoneal at <24 months age. From diagnosis last observation, 57 patients were fed on demand, 54 by NG...
