目的 探讨医院-社区-患者三级规范化管理对冠心病患者生存质量、生活习惯、用药依从性及预后的影响。 方法 将我院下属4个社区的所有行冠状动脉旁路移植术(CABG)或经皮冠状动脉介入治疗(PCI)的207例患者,按随机区组设计分为管理组和对照组。其中2个社区为管理组,对其社区医生进行培训,同时对其所在社区冠心病术后患者进行规范管理,另2个社区不进行医师培训和患者管理,作为对照组。对比两组患者的生存质量、生活习惯、用药依从性及预后。 结果 管理组患者通过医院与社区联合规范化管理后,心率、血压、血脂、体质指数等相关指标控制均比对照组好(均为P<0.05);同时,与对照组相比,管理组患者在低盐低脂饮食、合理休息、适当运动、戒烟戒酒等生活行为方面有所改善(均为P<0.05);两者在用药依从性方面,管理组(90.2%)明显优于对照组(55.8%,P=0.007);管理组患者的心原性死亡率(4.5%)较对照组低(8.4%,P=0.029)。 结论 通过医院与社区联合规范化管理冠心病术后患者,建立冠心病三级规范化管理体系,集医院-社区-患者于一体,能够提高冠心...
