Laparoscopic techniques have evolved, allowing increased capabilities within most subspecialties of general surgery, but failed to gain traction managing injured patients. We hypothesized that laparoscopy is effective in the diagnosis and treatment penetrating abdominal injuries. We retrospectively reviewed patients undergoing exploration following trauma at our Level 1 center during a 6-year period from January 1, 2008, December 31, 2013. Demographic resuscitation data were obtained...

BACKGROUND Patients with traumatic brain injury (TBI) are at high risk of venous thromboembolism events (VTE). We hypothesized that early chemical VTE prophylaxis initiation (≤24 hours a stable head CT) in severe TBI would reduce without increasing intracranial hemorrhage expansion (ICHE). METHODS A retrospective review adult patients 18 years or older isolated (Abbreviated Injury Scale score, ≥ 3) who were admitted to 24 Level I and II trauma centers from January 1, 2014 December 31 2020...

ABSTRACT BACKGROUND Despite the high incidence of blunt trauma in older adults, there is a lack evidence-based guidance for computed tomography (CT) imaging this population. We aimed to identify an algorithm guide use Pan-Scan (Head/C-spine/Torso) or Selective Scan (Head/C-spine ± Torso). hypothesized that patient’s initial history and exam could be used imaging. METHODS prospectively studied patients aged 65+ at 18 Level I/II centers. Patients presenting >24 h after injury who died upon...

A traumatic pseudoaneurysm of the superficial temporal artery is a rare vascular lesion that typically occurs after blunt trauma to region. It accounts for only 1% all aneurysms. These pseudoaneurysms need be appropriately diagnosed and treated without delay as patient can experience resulting symptoms severe headache, facial nerve palsy, arterial bleeding, and/or bone erosion. Diagnosis made with history along physical examination followed by confirmation ultrasound or computer tomography...

The majority of concussion research has focused on timely diagnosis and treatment, yet little been reported primary prevention. goal this study is to examine the use core training as a preventative tool. We performed non-randomized prospective involving high school athletes. They participated in 10-week session with exercises increasing strength [mobility, agility, stability, flexibility (MASSf)]. Logs were kept all concussions diagnosed using ImPACT testing. Statistical analysis was done...

Fellowship trainees in acute care surgery require experience the management of complex and operative trauma cases. Trauma center staffing usually follows standard 12-hour or 24-hour shifts, with resident fellow following a similar schedule. Although admissions can be generally unpredictable, we analyzed temporal trends patient arrival times to determine best time frame maximize trainee during each day.We reviewed 10 years (2007-2016) registry data for blunt penetrating activations. Hourly...

Extraskeletal Osteosarcoma (ESOS), a rare entity accounting for less than 2% of all soft tissue sarcomas. Known risk factors development include: middle aged and elderly patients, history radiation, controversial link to trauma. The typical presenting symptoms, if any, are tenderness swelling. In trauma these symptoms often mask the ESOS diagnosis assumed be hematoma or other traumatic diagnosis. Easy misinterpretation what appears obvious injury, can lead delays in accurate appropriate treatments.

Adiposis dolorosa, also known as Dercum's disease, is a rare disorder characterized by debilitating painful lipomas throughout the body. The prevalence and etiology of disease are unknown mentioned in National Organization Rare Disorders. We present an interesting case 53-year-old female who initially presented with six-week history subcutaneous masses. Ultrasound findings were suggestive lipomas, however, her symptoms beyond that benign lipomas. She then represented rapidly increasing...

Since the start of 2020, COVID-19 has profoundly impacted global health. Classically, disease manifests with pulmonary symptoms such as cough, dyspnea, pneumonia, and respiratory distress. However, it is important to note that in addition these symptoms, can present extrapulmonary symptoms. This case report involves an 82-year-old male who tested positive for causative agent - severe acute syndrome coronavirus 2 (SARS-CoV-2), whose presentation manifested gastrointestinal form colonic ileus....

Background Injured patients are at an increased risk of developing deep vein thrombosis (DVT) and pulmonary embolism (PE). Inferior vena cava (IVC) filters have been used in injured to prevent venous thromboembolism (VTE), but current evidence-based guidelines lacking. Methods Questions regarding IVC filter use with clearly defined Population(s), Intervention(s), Comparison(s), appropriately selected Outcomes (PICO) were formulated. One third of organ donors suffer catastrophic brain injury (CBI). There are no standard guidelines for the management traumatic CBI prior to death, and not all trauma centers have institutional guidelines. In addition, there is high variability in between institutions with Catastrophic vary may include various combinations hormone therapy, vasopressors, fluid resuscitation, other practices. We hypothesized that higher donation rates than those without. This prospective, observational...
