症例は24歳,男性.17歳時より上腹部の激痛を主訴に4回入院加療.2010年4月に同様の症状にて救急外来を受診し入院.腹部造影CTにて腸間膜基部に形成された異常なバンドを同定し,十二指腸水平脚の欠如・Treitz靱帯の無形成より腸回転異常症と診断した.このバンドが上腸間膜静脈枝を圧迫し,空腸が急激に鬱血・肥厚したことが腹痛の原因と考え,待機的に手術を施行した.腸間膜基部のバンド状の肥厚を切開し,空腸起始部周囲の自由度を確保した.術後の腹部造影CT・上部消化管造影では,腸間膜基部の異常バンドは消失し空腸壁の浮腫は認められず,十二指腸の走行は生理的走行に類似していた.現在術後2年だが,症状の再発なく良好に経過している.原因不明の腹痛を繰り返す症例には,腸回転異常を念頭に置いて精査を行い,腸回転異常症と診断した場合には積極的に手術を考慮する必要がある.
