症例は76歳,男性.抗凝固療法抵抗性の深部静脈血栓症(DVT)で循環器内科入院中に肉眼的血尿が出現し,当科紹介となった.血液検査では好酸球増多,血小板減少があり,尿細胞診はclass Iで尿中に好酸球を少数認めた.膀胱鏡では浮腫を伴う腫瘤性病変が広範に存在し,MRIでは筋層浸潤性膀胱癌を疑う所見であったため,経尿道的膀胱腫瘍切除術を施行した.病理組織学的検査の結果,悪性所見を認めず,多数の好酸球が膀胱間質に浸潤しており,好酸球性膀胱炎と診断した.好酸球性膀胱炎に対しプレドニゾロンを投与したところ,速やかに好酸球数は減少,血小板数は増加し,膀胱鏡にて腫瘤性病変の消失を確認した.投薬開始後8カ月現在,プレドニゾロン投与量を漸減中であるが,DVTは改善傾向であり,好酸球性膀胱炎の再発も認めていない.

症例は69歳,男性.1991年に原疾患不明の慢性腎不全に対し生体腎移植術を行ったが,2012年に巣状糸球体硬化症による移植腎機能低下に対し2回目の生体腎移植術を行い,当院腎臓外科で外来経過観察中であった.2019年4月肉眼的血尿を主訴に泌尿器科受診,膀胱鏡にて頂部右側壁寄りに2cm大の膀胱結石を認めた.膀胱結石に対し,レーザーによる経尿道的膀胱結石破砕術を施行.結石はレーザーにより砕石可能であったが,結石中心部に縫合糸を認め,縫合糸を核として結石が形成されたものと考えられた.術中縫合糸の除去はできなかったものの,術後6カ月現在結石の再発はなく,血尿も認めてない.
