失代偿期丙型肝炎肝硬化预后差,是否进行抗病毒治疗尚存争议.虽然抗病毒治疗应答率低且发生严重感染及死亡风险显著升高,但是通过清除HCV可延缓肝硬化进展,减少肝功能失代偿发生率,减少移植后丙型肝炎复发率,甚至可免于肝移植,显著改善患者预后[1 4].因此,美国肝病学会指南推荐该人群在严密监测下可进行抗病毒治疗[5].具体治疗方案,包括选择聚乙二醇干扰素(PEG-IFN)或普通IFN的剂量、疗程、最佳治疗时间以及如何最大限度提高获益/风险比等,尚未达成一致意见.本研究中我们用普通IFN小剂量逐渐加量(LADR)联合利巴韦林治疗失代偿期丙型肝炎肝硬化,评估治疗24周的疗效及安全性。
