目的 探讨伴MYD88L265P基因突变B细胞幼淋巴细胞白血病(B-PLL)的临床特点、诊断、鉴别诊断及治疗方案。 方法 回顾性分析江苏省盐城市第一人民医院1例伴MYD88L265P突变B-PLL患者的临床资料,并复习相关文献。 结果 患者为老年男性,临床主要表现为脾大、浆膜腔积液,根据血象、骨髓细胞学、免疫表型、基因突变检测(TP53、MYD88L265P等)明确诊断。采用苯丁酸氮芥、氟达拉滨、依布替尼等药物治疗。 结论 伴MYD88L265P突变的B-PLL临床罕见,易误诊、漏诊,Bruton酪氨酸激酶(BTK)抑制剂的使用为B-PLL治疗带来希望。
