【目的】入院患者のための麻薬自己管理システムを構築し, その有用性を検討した. 【方法】院内医療スタッフへのアンケート結果をふまえ, 多職種によるワーキンググループで麻薬自己管理の患者条件と方法を定めた. 対象は, (1)内服薬自己管理アセスメント適合, (2)麻薬自己管理アセスメント適合, (3)医師の同意あり, (4)患者の希望あり, の4条件を満たした患者とした. また, 自己管理を実施した患者および医療スタッフにアンケートを行った. 【結果】2008年4月より12カ月間に内外用の麻薬施用の入院患者は100名おり, 麻薬自己管理可能患者は26名, そのうち患者希望で実施したのは20名であった. 自己管理中(平均15.0日間)に紛失, 盗難, 誤服用はなかった. 実施患者の94％は, 手元に薬があって安心, 困ったことはないと答えた. スタッフは75％が自己管理を継続すべきと答えた. 【結語】当システムは病棟での安全で適切な麻薬自己管理実施に有用と示唆された.
