症例はアルコール肝硬変を既往にもつ64歳の男性で,右下腹部痛と血便のため前医を受診され,CTで終末回腸に壁肥厚を指摘され当院紹介となった.カプセル内視鏡で,中部から下部小腸を中心に頂部発赤調のSMT様隆起の多発を認め,ダブルバルーン内視鏡を用いて病変より生検を行い,回腸に多発する神経内分泌腫瘍(Neuroendocrine tumor;以下,NETと略記)の診断となり手術目的に当科を紹介受診された.術中所見ではTreitz靭帯より315cmから365cmに多発する腫瘍を認め,同部は小腸切除を施行した.また,術中内視鏡で同部以外の離れた6ヵ所にも腫瘍を認め,小腸切除後の栄養障害および肝機能増悪を回避するため,それぞれ楔状に局所切除を施行した.術後の病理診断で腫瘍はすべてNET(G1,G2)の診断であった.多発する小腸NETは本邦では比較的稀な疾患であるため,若干の考察を加えて報告する.
