Hyponatremia often reflects a free water excess. Sodium/glucose cotransporter 2 (SGLT2) inhibitors increase excretion through glucose-induced osmotic diuresis. In this multi-centre cohort of 156 patients with hypotonic polyuria, direct measurement hypertonic saline-stimulated plasma copeptin had much greater diagnostic accuracy than a standard water-deprivation test, as judged by the final reference diagnosis, which was determined on basis medical history, test results (with levels masked), and treatment response at 3-month follow-up visit.
