A useful testing strategy is proposed for a confirmatory phase III clinical trial. It consists of combined test superiority and equivalence, it easy to apply. By introducing the strategy, we can perform post hoc analysis in experiment. Thus more flexible decision will be possible than usual single method provides. shown that procedure needs no adjustment multiplicity from point view closed procedure. The relationship between this confidence interval also discussed.
Only a few studies have documented the characteristics of inflammatory bowel disease (IBD) associated with primary sclerosing cholangitis (PSC). We aimed to clarify clinical and histopathological IBD PSC (PSC-IBD).Twenty-nine patients 60 ulcerative colitis (UC) but without complicating were enrolled in this study. First, age sex distribution, affected area, course, number recurrent attacks, severity UC investigated. Then, mucosal specimens obtained from right side (cecum ascending colon),...
Pancreatitis and cholecystitis are major complications after self-expandable metal stent (SEMS) placement in distal malignant biliary obstruction. We aimed to clarify predictive factors for pancreatitis covered SEMS placement. We retrospectively reviewed 74 consecutive patients with obstruction who underwent initial endoscopic drainage using SEMS. Predictive were evaluated the described above 66 had not undergone cholecystectomy. The incidences of 10.8% (8/74) 6.1% (4/66), respectively....
The diagnostic yields of endoscopic transpapillary brush cytology and forceps biopsies for malignant biliary strictures (MBS) remain unclear predictive factors diagnosis have not been established. We aimed to clarify the both methods factorsWe reviewed 241 patients with who underwent (n = 202) or biopsy (n= 208) between 2004 2014 at a single academic center. The sensitivity MBS was significantly higher than that [60.6% (97/160) vs 36.1% (57/158), P < 0.01). in diagnosing bile duct cancer...
To clarify the frequency and clinical significance of extrapancreatic lesions in autoimmune pancreatitis (AIP).The characteristics during course AIP were investigated retrospectively 64 patients with AIP. The predictive factors for relapse at onset also examined.Extrapancreatic occurred 95% (61/64) frequencies sclerosing cholangitis, sialadenitis, retroperitoneal fibrosis, mediastinal or hilar lymphadenopathy 84% (54/64), 23% (15/64), 16% (10/64), 77% (27/35), respectively. Patients...
Pancreatojejunostomy stricture is one of the complications after pancreaticoduodenectomy [1]. Endoscopic ultrasound-guided pancreatic duct drainage (EUS-PD) has emerged as an alternative to enteroscopy-guided treatment. We present a case demonstrating successful coordinated manipulation 3-Fr microcatheter and pancreatoscopy for recanalization complete pancreaticojejunostomy obstruction.
Abstract Background Robotic needle insertion under CT guidance has been developed, but data on comparison with manual are still lacking. Purpose To compare robotic versus for fluoroscopy-guided intervention, primarily in terms of accuracy. Materials and Methods This was a prospective study between May 2020 March 2023 at single site. The cohort comprised 22 patients undergoing (Aquilion One or Aquilion CX; Canon Medical Systems) biopsy, who were randomly allocated to either the group. robot...
Autoimmune pancreatitis (AIP) is often associated with systemic extrapancreatic lesions, and the concept of IgG4-related autoimmune disease has been proposed. We report a 62-year-old woman AIP various lesions during long-term clinical course. These occurred after withdrawal steroid therapy. Sclerosing sialadenitis before AIP, whereas sensorineural hearing loss, allergic purpura endocapillary proliferative glomerulonephritis AIP. However, no relapse was observed for 5 years start maintenance...
Comparisons of intraductal ultrasonography (IDUS) findings between primary sclerosing cholangitis (PSC) and IgG4-related (IgG4-SC) have not been elucidated. We aimed to clarify the differences in transpapillary IDUS PSC IgG4-SC.We retrospectively compared 15 patients with 35 IgG4-SC 2004 2014.IDUS circular-asymmetric wall thickness, irregular inner margin, diverticulum-like outpouching, unclear outer heterogeneous internal echo, disappearance three layers were significantly higher than (P <...
Although endoscopic transpapillary gallbladder drainage (ETGBD) has been reported as an alternative procedure for acute cholecystitis, it requires advanced techniques. In terms of the certainty achieving drainage, remains a challenging procedure. The aim current study was to elucidate practical efficacy cholangioscopic assistance and develop new classification that could be used evaluate technical difficulty ETGBD provide theoretical strategy apply cholangioscopy appropriately difficult...
Abstract Various biomarkers have been proposed for sepsis; however, only a few become the standard. We previously reported that plasma histidine-rich glycoprotein (HRG) levels decreased in septic mice, and supplemental infusion of HRG improved survival mice model sepsis. Moreover, our previous clinical study demonstrated patients were lower than those noninfective systemic inflammatory response syndrome patients, it could be biomarker In this study, we focused on assessed differences between...
Autoimmune pancreatitis (AIP) needs to be differentiated from pancreatic cancer (PC). We aimed clarify the findings specific for AIP by comparing clinical differences between mass-forming and PC.We retrospectively compared 36 patients with 60 PC without metastasis regarding clinical, imaging, serological, histological other organ involvement (OOI). evaluated sensitivity, specificity accuracy of these differential diagnosis PC.The 100% were a capsule-like rim on computed tomography (CT),...

Objectives. Autoimmune pancreatitis (AIP) responds well to corticosteroid therapy (CST), and CST is essential induce remission. However, the correlation between long-term outcome has not been evaluated. We aimed clarify of AIP CST. Material methods. retrospectively evaluated relapse, risk malignancy side effects by focusing on with in 84 patients type 1 AIP. Results. The incidence relapse was 23.8%. frequency after administration significantly lower taking for >6 months than those who did...
Abstract Background and Aim The opportunities of endoscopic retrograde cholangiopancreatography ( ERCP )‐related procedure for hemodialysis HD ) patients have been increasing recently. However, the complication rate ERCPs in has not evaluated sufficiently. We aimed to clarify feasibility patients. Methods retrospectively reviewed 76 consecutive between J anuary 2005 D ecember 2012 one university hospital three tertiary‐care referral centers. Endoscopic sphincterotomy EST was performed 21...
The revised Atlanta classification is widely used for the evaluation of acute pancreatitis (AP) severity. However, this cannot be within 48 hours AP onset. aim study was to investigate predictive factors mortality in patients with on admission. We evaluated association between and clinical parameters at time admission from April 2013 December 2017 one university hospital tertiary care referral center.A total 203 consecutive were enrolled. Nine (4.4%) died despite multidisciplinary treatment....
Background JME-001 is a phase II trial assessing the efficacy and safety of cisplatin, pemetrexed, nivolumab as first-line therapy in malignant pleural mesothelioma (MPM). Patients methods with untreated, unresectable MPM an Eastern Cooperative Oncology Group (ECOG) performance status (PS) 0–1 were included. The primary endpoint centrally reviewed objective response rate. secondary endpoints include (1) rate assessed by investigators, (2) disease control rate, (3) overall survival, (4)...
Endoscopic ultrasonography (EUS)-guided ethanol injection (EI) has recently been introduced as one of the management strategies for pancreatic neuroendocrine neoplasms (PNENs). However, its role a surgical alternative is unclear. We evaluated efficacy and safety EUS-EI in treating small PNENs through prospective multicentre study.
