Solid organ transplant recipients may be at a high risk for SARS-CoV-2 infection and poor associated outcomes. We herein report our initial experience with solid two centers during the first 3 weeks of outbreak in New York City. Baseline characteristics, clinical presentation, antiviral immunosuppressive management were compared between patients mild/moderate severe disease (defined as ICU admission, intubation or death). Ninety analyzed median age 57 years. Forty-six kidney recipients, 17...
Objective: To assess the clinical utilization and performance of FilmArray® Meningitis/Encephalitis (ME) multiplex polymerase chain reaction (PCR) panel in a hospital setting. Background: Rapid diagnosis treatment central nervous system (CNS) infections are critical to reduce morbidity mortality. The ME is Food Drug Administration (FDA) approved rapid PCR assay that targets 14 bacteria, viruses, fungi. Previous studies show an overall agreement 93-99% between conventional diagnostic testing....
A review of 15 patients who tested positive for human herpesvirus 6 (HHV-6) on the FilmArray Meningitis/Encephalitis panel revealed that majority were unlikely to have HHV-6 encephalitis. Criteria assist interpretation results are presented.
Abstract We report 2 patients with invasive aspergillosis after infection pandemic (H1N1) 2009. Influenza viruses are known to cause immunologic defects and impair ciliary clearance. These defects, combined high-dose corticosteroids prescribed during influenza-associated adult respiratory distress syndrome, may be novel risk factors predisposing otherwise immunocompetent aspergillosis.
Multidrug-resistant organisms (MDROs) are an important cause of morbidity and mortality after solid organ transplantation. We aimed to characterize MDRO colonization dynamics infection in liver transplant (LT) recipients through innovative use active surveillance whole-genome sequencing (WGS). prospectively enrolled consecutive adult patients undergoing LT from March 2014 2016. Fecal samples were collected at multiple timepoints time enrollment 12 months posttransplant. Samples screened for...
The safety and efficacy of tocilizumab for the treatment severe respiratory symptoms due to COVID-19 remain uncertain, in particular among solid organ transplant (SOT) recipients. Thus, we evaluated clinical characteristics outcomes 29 hospitalized SOT recipients who received COVID-19, compared a matched control group did not. Among total 117 with (24.8%) tocilizumab. 90-day mortality was significantly higher patients (41%) those not (20%, P = .03). When by age, hypertension, chronic kidney...
Abstract: In 2022–2023 mpox outbreak led to declaration of a public health emergency. Smallpox vaccination has increased immunity and decrease in clinical disease. The role immunosuppressed patients is not well understood. We present an atypical case cutaneous mpox, presenting fully vaccinated, immunosuppressed, HIV-negative patient as initial isolated, subcutaneous neck mass. review the diagnostic dilemma workup for these cases.
ABSTRACT While much is known about the geographic distribution of different clonal types methicillin-resistant Staphylococcus aureus (MRSA), few studies have assessed molecular epidemiology methicillin-susceptible S. (MSSA), despite its continued clinical importance. In each U.S. Census region, reference laboratories collected successive MSSA isolates from patients with invasive or superficial staphylococcal infections for use in Tigecycline Evaluation and Surveillance Trial. All periods...
Understanding the changing epidemiology of Staphylococcus aureus bacteremia, as well variables associated with poor outcomes, can yield insight into potential interventions. This study was a retrospective, observational cohort adult patients at an academic medical center in New York City who had S. bloodstream infections between 1 January 2007 and 31 December 2015. Participants were divided 3 periods: group (2007-2009), 2 (2010-2012), (2013-2015) for trend analysis. All clinical strains...
The methicillin-susceptible Staphylococcus aureus (MSSA) clone sequence type (ST) 398 has increasingly been identified as a pathogen in diverse geographic settings, yet its epidemiology remains incompletely understood. In this case-control study of MSSA infections, we ST398 both major community- and hospital-associated the Dominican neighborhood northern Manhattan.
Obesity increases a person's susceptibility to variety of infections, including Staphylococcus aureus which is an important cause morbidity in correctional settings. Using cross-sectional design, we assessed the association between obesity and S. colonization, risk factor for subsequent infection, New York State maximum-security prisons (2011–2013). Anterior nares oropharyngeal cultures were collected. Structured interviews medical records used collect demographic, behavioral, data. Body...
Background Tocilizumab, an interleukin-6 receptor blocker, has been used in the inflammatory phase of COVID-19, but its impact independent corticosteroids remains unclear patients with severe disease. Methods In this retrospective analysis COVID-19 admitted between March 2 and April 14, 2020 to a large academic medical center New York City, we describe outcomes associated tocilizumab 400 mg (without methylprednisolone) compared propensity-matched control. The primary endpoints were change...
There is an ongoing need to understand whether transplantation during acute Coronavirus disease 2019 (COVID-19) can be performed safely, especially when urgent transplant required. We collected retrospective data of all consecutive non-lung recipients who had a positive SARS-CoV-2 polymerase chain reaction (PCR) on the day planned deceased donor organ implantation. Data were from two large centers 01/01/2022 02/01/2023. Demographics, details regarding COVID-19 infection, waitlist priority,...
BackgroundChildren with short bowel syndrome (SBS) suffer from strikingly high rates of morbidity and mortality, due in part to their susceptibility life-threatening infectious diseases. Few large, multisite studies have evaluated patient-specific factors associated bacteremia hospitalized children without SBS.
Extracorporeal membrane oxygenation (ECMO) is a means of life support for failing patients who require extreme life-saving measures due to failure their heart, lungs or both organs. In patient suffering cardiac arrest, the faster circulation via cardiopulmonary resuscitation (CPR) can be instituted better outcome is. If an ECMO circuit needs built and primed it may add significant minutes response time. The purpose this study test any growth in circuits at given time intervals prove safety...
Abstract Background Discard rate of Public Health Service Increased Risk (PHS‐IR) organs is high despite the absence worse kidney transplant outcomes. Methods We conducted a retrospective, single‐center study PHS‐IR offers made to transplant‐only potential recipients from 6/2004 5/2015. Overall mortality and outcomes between were stratified by response offers. Cox regression Kaplan‐Meier analyses allograft failure performed. Results A total 2423 offered kidney, with 1502 transplanted, or...
Abstract Few treatment options are available for oseltamivir‐resistant influenza. It has been proposed that baloxavir can be effective in this setting due to its distinct mechanism of action but clinical experience is lacking immunocompromised patients. We report two such cases treated with after failure oseltamivir and detection resistance mutations. Baloxavir/zanamivir combination therapy was one patient, persistent viral shedding noted monotherapy the other patient.
The optimal duration of transmission-based precautions among immunocompromised patients with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is unknown.Retrospective review solid organ transplant positive SARS-CoV-2 polymerase chain reaction result from nasopharyngeal specimens admitted to the hospital between March 13, 2020 and May 15, 2020.Twenty-one percent recipients detected ≥20 d after symptom onset (or first test asymptomatic individuals) had a low cycle threshold (ie,...
Unlike immunocompetent hosts, the duration of viral persistence after infection with severe acute respiratory syndrome coronavirus 2 can be prolonged in immunosuppressed patients. Here, we present a case for over 19 weeks patient history solid organ transplant and explore clinical, virologic, immunologic course. Our still demonstrated at 138 days low polymerase chain reaction cycle threshold values evidence continuing sequence evolution indicative ongoing virus replication. These findings...
To assess applicability of adult quality indicators in the management pediatric community-acquired pneumonia (CAP), a retrospective cohort study children admitted with CAP was conducted. The primary outcome achievement recommended indicators. Associations between and clinical outcomes were assessed. Most had rapid oxygenation assessment (97%) blood culture before antibiotics (98%). Antibiotic timing selection met less frequently. only indicator associated time to stability or length stay...
