The recent decline in available personal protective equipment (PPE) due to the novel coronavirus (COVID-19) pandemic has given rise a host of three-dimensional (3D) printed prototypes for facemask and respirator units. Many these models have been made open access publicly printing use, promoted by various media outlets. Although desktop 3D measures provided possible venue success providing homemade cost-effective PPE health care workers, rapid dissemination performed without reproducible...
Vasoplegic syndrome is a well-recognized complication during cardiopulmonary bypass (CPB) and associated with increased morbidity mortality, especially when refractory to conventional vasoconstrictor therapy. This the first reported case of vasoplegia on CPB unresponsive methylene blue whereas responsive hydroxocobalamin, which indicates that effect hydroxocobalamin outside nitric oxide system significant or two drugs have synergistic in one multiple mechanisms.
Lung transplantation has been evolving since its inception in 1963. Over recent years, literature suggested a shift the perioperative strategy of mechanical support toward extracorporeal membrane oxygenation (ECMO) as preferred modality life (ECLS) lung transplantation. The Survey ECLS Application (SEAL) was designed to elucidate current practice patterns within United States. In this cross-sectional study, physician from 62 adult centers across States America (USA) surveyed on their...
Abstract Background Primary graft dysfunction (PGD) after lung transplantation (LuTx) contributes substantially to early postoperative morbidity. Both intraoperative transfusion of a large amount blood products during the surgery and ischemia–reperfusion injury allograft implantation play an important role in subsequent PGD development. Methods We have previously reported randomized clinical trial 67 patients where point care (POC) targeted coagulopathy management administration 5% albumin...
Extracorporeal circulation (ECC) support using intraoperative extracorporeal membrane oxygenation (ECMO) during lung transplantation (LTx) is now a routine practice for many high volume centers. Circuits that are dedicated to ECMO alone can be expensive and do not allow full cardiopulmonary bypass (CPB) performed. We describe our technique of instituting venoarterial LTx less-expensive hybrid circuit facilitates easy immediate conversion CPB if needed, without interruption ECC.
Abstract Background Surgery is the primary treatment for non-small cell lung cancer (NSCLC), but microscopic residual disease may be unavoidable. Preclinical studies have shown that volatile anesthetics might suppress host immunity and promote a pro-malignant environment supports proliferation, migration, angiogenesis, whereas propofol preserve cell-mediated inhibit tumor angiogenesis. However, clinical evidence propofol-based total intravenous anesthesia (TIVA) can reduce recurrence after...
BackgroundThe utilization of extracorporeal life support (ECLS) for intraoperative during lung transplantation has increased over the past decade. Although veno-arterial membrane oxygenation (VA-ECMO) recently emerged as preferred modality cardiopulmonary bypass (CPB), many centers continue to use both forms ECLS transplantation. Our novel hybrid VA-ECMO/CPB circuit allows seamless transition from VA-ECMO CPB at a significant cost savings compared standalone circuit. This study describes our...
The clinical, educational, and research facets of lung transplantation have advanced significantly since the first transplant in 1963. formation International Society for Heart Lung Transplantation (ISHLT) subsequent Registry has forged a precedent collaborative teamwork that affected current outcomes. Advancement Anesthesia (SATA) is dedicated to developing educational platforms all anesthesia. Additionally, we believe anesthetic training not kept pace with other advances field. As such,...
Background The use of extracorporeal circulation (ECC) for intraoperative cardiopulmonary support during lung transplantation has been increasing in the recent years. Our group previously described a novel hybrid membrane oxygenation (ECMO) circuit transplantation. Technique approach management our ECMO is driven by two main goals: first to deliver that ensures an appropriate balance between native and cardiac outputs order provide stable environment promotes attenuation ischemic-reperfusion...
This review highlights key studies examining perioperative management of cardiothoracic transplantation published in 2023. Articles were manually screened after searching Scopus, PubMed, and Google Scholar databases for manuscripts related to transplantation, which yielded 343 papers with 15 qualitatively selected as the most salient readers. Overarching themes include differences outcomes across various etiologies end-stage lung disease, novel developments expand donor pool, multi-organ...
