Mechanical thrombectomy (MT) has revolutionized the treatment of acute ischemic stroke (AIS) due to large vessel occlusion (LVO), but its efficacy and safety in medium (MeVO) remain less explored. This multicenter, retrospective study aims investigate incidence clinical outcomes perforations (confirmed by extravasation during an angiographic series) MT for AIS caused MeVO.
Acute ischemic stroke (AIS) caused by distal medium vessel occlusions (DMVOs) represents a significant proportion of overall cases. While intravenous thrombolysis (IVT) has been primary treatment, advancements in endovascular procedures have led to increased use mechanical thrombectomy (MT) DMVO patients. However, symptomatic intracerebral hemorrhage (sICH) remains critical complication AIS, particularly after undergoing intervention. This study aims identify factors associated with sICH patients MT.
(a) To characterize the frequency of objective cognitive deficits and self-perceived difficulties (b) to explore demographic clinical predictors dysfunction complaints.
Background The efficacy of endovascular treatment (EVT) in acute ischaemic stroke due to distal medium vessel occlusion (DMVO) remains uncertain. Our study aimed evaluate the safety and EVT compared with best medical management (BMM) DMVO. Methods In this prospectively collected, retrospectively reviewed, multicentre cohort study, we analysed data from Multicentre Analysis primary Distal occlusions: effect Mechanical Thrombectomy registry. Patients DMVO M2, M3 M4 segments who underwent or...
Stroke remains a major health concern globally, with oral anticoagulants widely prescribed for stroke prevention. The efficacy and safety of mechanical thrombectomy (MT) in anticoagulated patients distal medium vessel occlusions (DMVO) are not well understood.
Acute ischemic stroke (AIS) from primary medium vessel occlusions (MeVO) is a prevalent condition associated with substantial morbidity and mortality. Despite the common use of mechanical thrombectomy (MT) in AIS, predictors poor outcomes MeVO remain poorly characterized.
Background The combination of intravenous thrombolysis (IVT) with mechanical thrombectomy (MT) may have clinical benefits for patients medium vessel occlusion. Purpose To examine whether MT combined IVT is associated different outcomes than alone in acute ischemic stroke (AIS) and Materials Methods This retrospective study included consecutive adult AIS occlusion treated or at 37 academic centers North America, Asia, Europe. Data were collected from September 2017 to July 2021. Propensity...
Background and Purpose The management of acute ischemic stroke (AIS) due to distal medium vessel occlusion (DMVO) remains uncertain, particularly in comparing the effectiveness intravenous thrombolysis (IVT) plus mechanical thrombectomy (MT) versus IVT alone. This study aimed evaluate safety efficacy DMVO patients treated with either MT-IVT or alone. Methods multinational analyzed data from 37 centers across North America, Asia, Europe. Patients AIS were included, collected September 2017...
Introduction: First pass effect (FPE), achievement of complete recanalization (mTICI 2c/3) with a single pass, is significant predictor favorable outcomes for endovascular treatment (EVT) in large vessel occlusion stroke (LVO). However, data concerning the impact on functional and predictors FPE medium occlusions (MeVO) are scarce. Patients Methods: We conducted an international retrospective study MeVO cases. Multivariable logistic modeling was used to establish independent FPE. Clinical...

Background and Objectives
COVID-19–related inflammation, endothelial dysfunction, coagulopathy may increase the bleeding risk lower efficacy of revascularization treatments in patients with acute ischemic stroke (AIS). We aimed to evaluate safety outcomes AIS COVID-19.
Methods
This was a retrospective multicenter cohort study consecutive receiving intravenous thrombolysis (IVT) and/or endovascular treatment (EVT) between March 2020 June 2021 tested for severe respiratory...
Background For acute proximal intracranial artery occlusions, contact aspiration may be more effective than stent‐retriever for first‐line reperfusion therapy. Due to the lack of data regarding medium vessel occlusion thrombectomy, we evaluated outcomes according technique in a large, multicenter registry. Methods Imaging, procedural, and clinical patients with occlusions (M2, A1, or P1) distal (M3, A2, P2, further) treated at 37 sites 10 countries were analyzed endovascular (stent‐retriever...
Despite the proven effectiveness of endovascular therapy (EVT) in acute ischemic strokes (AIS) involving anterior circulation large vessel occlusions, isolated posterior cerebral artery (PCA) occlusions (iPCAo) remain underexplored clinical trials. This study investigates comparative and safety EVT against medical management (MM) patients with iPCAo.
Background: Medium vessel occlusion (MeVO) strokes, particularly affecting the M2 segment of middle cerebral artery, represent a critical proportion acute ischemic posing significant challenges in management and outcome prediction. The efficacy mechanical thrombectomy (MT) MeVO stroke may warrant reliable predictors functional outcomes. This study aimed to investigate prognostic value follow-up infarct volume (FIV) for predicting 90-day outcomes patients undergoing MT. Methods: multicenter,...
Background Current randomized controlled trials are investigating the efficacy and safety of mechanical thrombectomy (MT) in patients with medium vessel occlusion (MeVO) stroke. Whether best medical management (MM) is more efficient than unsuccessful recanalization during MT remains unknown. Methods This was a retrospective cohort study using data from 37 academic centers across North America, Asia, Europe between September 2017 July 2021. Only distal branches (M2 M3) middle cerebral artery...
Acute ischemic stroke (AIS) from distal medium vessel occlusion (DMVO) presents unique treatment challenges. Mechanical thrombectomy (MT) is emerging as a viable option for these patients, yet the role of pre-stroke aspirin unclear. This study evaluates impact low-dose on outcomes in DMVO patients undergoing MT. We conducted multinational, multicenter, propensity score-weighted analysis within Multicenter Analysis primary Distal occlusions: effect Thrombectomy (MAD-MT) registry. Patients...
Mechanical thrombectomy (MT) is established as an effective treatment for large vessel occlusion strokes, but its efficacy and safety medium occlusions (MeVOs) remain less clear. This study examines the impact of periprocedural embolization to a new vascular territory (ENT) on clinical outcomes in patients with MeVO stroke treated MT. A multicenter, retrospective analysis was conducted using MAD-MT (Multicenter Analysis primary Distal occlusions: effect Thrombectomy) registry, including 2122...
Background Endovascular therapy (EVT) for stroke due to distal or medium vessel occlusion (DMVO) is safe. Due the distinct anatomical characteristics of DMVOs, further evaluation EVT crucial determine which devices may yield better outcomes. Methods A retrospective analysis adults with DMVO treated in 37 centers (11 countries) was queried. The primary outcome favorable shift 90-day modified Rankin Scale (mRS) compared between patients Trevo versus other on first pass using 1:1 propensity...
Early outcome prediction after acute ischemic stroke (AIS) might be improved with blood-based biomarkers. We investigated whether the longitudinal profile of a multi-marker panel could predict successfully recanalized AIS patients. We used ultrasensitive single-molecule array (Simoa) to measure glial fibrillary acidic protein (GFAP), neurofilament light chain (NfL), total-tau (t-tau) and ELISA for brevican in prospective study patients anterior circulation large vessel occlusion submitted...
