2004年~2016年で成人無菌性髄膜炎437例中,水痘・帯状疱疹ウイルス(varicella zoster virus; VZV)髄膜炎は11例(52.7 ± 14.9歳,すべて男性),頻度は2.5%であった.夏~秋に多く地区の水痘の流行と鏡像的であった.9例は髄液中VZV-DNA陽性,2例はVZV-IgG抗体価指数2以上で診断した.発症7日頃までは前者,以後は後者が有用であった.8例は帯状疱疹が先行,1例は髄膜炎が先行,2例は皮疹を認めなかった.2例はB型肝炎キャリアー,1例はインフルエンザワクチン注射後,1例がステロイド内服中であった.神経合併は三叉神経痛,顔面神経麻痺,舌咽神経痛,Elsberg症候群など認め重症化する症例もあった.
症例は69歳の男性である.入院10日前頃から左下肢に帯状疱疹(herpes zoster; HZ)が出現し,その後四肢痙性運動不全麻痺を発症した.皮疹は左腰髄L2~4領域と全身にも散在していた.脊髄MRIで延髄下端~胸髄Th2の長軸に長いT2強調像で高信号病巣を示した.髄液蛋白増加,リンパ球優位の細胞増加を示し,水痘・帯状疱疹ウイルス(varicella-zoster virus; VZV)DNAが陽性であった.血清抗myelin oligodendrocyte glycoprotein(MOG)抗体が陽性で抗aquaporin4抗体は陰性であった.肺炎と肝障害も認め,皮疹の状況から汎発性HZと思われた.HZが引き金となり,抗MOG抗体が産生されて,脊髄炎が発症した病態が推察された.
