要旨:症例は初産の37 歳女性.妊娠の経過は良好であったが分娩時に頭痛・左上下肢麻痺の出現を認めたため転院搬送された.頭部CT で右基底核領域の脳出血を認め,降圧加療を行い全身麻酔下で緊急帝王切開術を施行した.第14 病日の頭部MRI で頭蓋内血管に多発狭窄が見られreversible cerebral vasoconstriction syndrome(RCVS)と診断した.Transcranial color flow imaging(TC-CFI)では狭窄部に一致した流速の上昇を認めた.神経症状は徐々に改善し,MRI とTC-CFI で狭窄部の改善度を経時的に評価しえた.MRI と同様に,TC-CFI はRCVS の血管病変の継時的な評価が可能である.
