症例は67歳, 女性. 平成12年10月の健康診断時に心電図においてI・aVL・V1~V4誘導に陰性T波を認めたため, 精査加療目的で, 11月10日に当科外来を受診した. 諸検査の結果, 肥大型非閉塞性心筋症と診断し, 以後当科外来で経過観察していた. 外来受診時には, すでに陰性T波は改善していた. 平成15年9月上旬に, 胸部不快感を自覚した. 平成15年9月29日の心電図において, I・II・III・aVL・aVF・V2~V6誘導で陰性T波を認めた. 虚血性変化を疑い, 10月7日に当科へ入院した. 心臓カテーテル検査では, 冠動脈に有意な狭窄は認められなかった. MIBG心筋シンチにおいて, 心尖部を中心に集積の低下を認めたため, たこつぼ型心筋障害による心電図変化所見と判断した. その後の経過で陰性T波はほぼ消失した. 今回, たこつぼ型心筋障害によると思われる心電図変化を繰り返した肥大型非閉塞性心筋症の1例を経験したので報告する.
