症例は58歳の男性で,2012年7月胃癌に対して開腹幽門側胃切除術を施行した.病理所見はL,Less,tub2>por1,pT1b(SM2)N0M0,pStage IAであり,術後は経過観察を行った.術後12か月のフォローCTにて膵頭部腫瘤性病変を認め局所再発と診断した.腫瘍マーカーの上昇は認めず,画像上その他の再発所見は認めなかったため,2013年8月開腹膵頭十二指腸切除+肝部分切除+横行結腸部分切除を施行した.再手術後,術後補助化学療法として1年間S-1の内服を行い,その後は経過観察を継続し現在再発切除後9年間,無再発生存中である.局所に限局しており,根治的切除が可能であれば,胃癌局所再発に対しても手術も治療の選択肢になると考えられた.
