我々は、出生後けいれん重積を繰り返した結節性硬化症の2カ月女児に対し、間欠期および発作時脳波で焦点と考えられたC3領域の皮質結節および周囲の焦点切除術を施行した。術中脳波で記録された2回のelectrical seizureに対し、術後にDirect Current shift(DC shift)を含むwide band analysisを行い、結節性硬化症のてんかん原性について考察した。結節周囲では発作時にspikeを認め、一部でhigh frequency oscillation(HFO)も確認できたが、ictal DC shiftは認めなかった。一方、結節部位では発作時spikeやHFOは認めなかったが、2度ともictal shiftを認めた。本症例では、発作時spikeの観察のみではてんかん原性なしと判断される結節部位にwide analysisを行うことで、結節自体にもてんかん原性が有る可能性が示唆された。
