We performed a prospective, randomized study in patients with previously untreated advanced (Stage III or IV) laryngeal squamous carcinoma to compare the results of induction chemotherapy followed by definitive radiation therapy those conventional laryngectomy and postoperative radiation.
Asymptomatic ventricular arrhythmias in patients with congestive heart failure are associated increased rates of overall mortality and sudden death. Amiodarone is now used widely to prevent tachycardia fibrillation. We conducted a trial determine whether amiodarone can reduce asymptomatic arrhythmias.
A sudden increase in the incidence of post-transplantation lymphoproliferative disorder among patients our cardiac-transplantation program was temporally related to introduction immunosuppressive drug OKT3. This monoclonal antibody has come be widely used recent years both prevent and treat rejection after cardiac transplantation.
Background —In a multicenter, double-blind, placebo-controlled study, the long-term effects of amiodarone on morbidity and mortality in patients with congestive heart failure (CHF) atrial fibrillation (AF) were evaluated during 4-year period. Methods Results —Of 667 CHF, 103 (15%) had AF at baseline. Of these, 51 randomized to 52 placebo. The group sinus rhythm comparable except for higher proportion nonischemic versus ischemic cardiomyopathy (41% 27%, P <0.005). mean ventricular response...
Clostridium difficile infection (CDI) is a major cause of hospital-acquired diarrhea and most commonly associated with changes in normal intestinal flora caused by administration antibiotics. Few studies have examined the risk CDI total dose, duration, or number antibiotics while taking into account complex exposures over time.A retrospective cohort study conducted from 1 January to 31 December 2005 among hospitalized patients 18 years older receiving 2 more days antibiotics. The identified...

Objective
To assess long-term quality of life in surviving patients with advanced laryngeal cancer.
Design
A follow-up quality-of-life survey randomized to the Veterans Affairs Laryngeal Cancer Study No. 268 on induction chemotherapy and radiation (CT + RT) vs surgery RT.
Setting Patients
Forty-six (71%) 65 prior stage III or IV cancer who could be contacted completed survey: 25 from RT group 21 CT group. Baseline demographic clinical characteristics among...
Few studies have evaluated the independent effect of a cancer diagnosis on vulnerability and frailty, which been associated with adverse health outcomes in older adults. We used data 2003 Medicare Current Beneficiary Survey from nationally representative sample 12,480 community-dwelling elders. Multivariable logistic regression models were to evaluate whether was independently frailty. Measures frailty included disability, geriatric syndromes, self-rated health, scores two assessment tools...
Heparin-induced thrombocytopenia with thrombosis (HITT) can lead to serious morbidity and may be potentially fatal. We reviewed our experience this entity over a 4-year period, determine the following: 1) incidence type of in patients heparin-induced (HIT), 2) clinical consequences thrombosis, i.e., amputation, cerebrovascular accidents death, 3) risk factors associated development 4) impact therapy on outcomes HITT. Between 1991-1994, 108 were diagnosed have HIT by platelet aggregation...
Background Although trials of amiodarone therapy in patients with congestive heart failure have produced discordant results regard to effects on survival, most studies reported a significant rise left ventricular ejection fraction during long-term therapy. In the present study, we determined whether this increase is associated an improvement symptoms and/or physical findings or reduction number hospitalizations for failure. Methods and Results Department Veterans Affairs cooperative study...
Veterans of the US military service constitute a dynamic cohort that is suitable for epidemiologic investigations. However, ability to ascertain vital status paramount value this population in studies having mortality as primary endpoint. The purpose study was estimate and compare ascertainment obtained from Department Affairs Patient Treatment File Beneficiary identification Record Locator System (GIRLS) with National Death Index on 17,118 male veterans hospitalized 1970 or 1971. Based...
In Brief OBJECTIVE: To assess the effect of low-dose mifepristone on quality life, pain, bleeding, and uterine size among women with symptomatic leiomyomata. METHODS: Forty-two leiomyomata volume 160 mL or more were randomized to mifepristone, 5 mg daily, placebo for 26 weeks. Quality life (Uterine Fibroid Symptoms Life Questionnaire Medical Outcomes Study 36-Item Short Form survey) leiomyoma (ultrasonography) assessed at baseline, 1 month, 3 months, 6 months treatment. Bleeding (daily logs...

Objective
To determine the longitudinal effect of abuse by an intimate partner on risk behavior among adolescents.
Design and Participants
Linear regression analyses data from 4443 adolescents surveyed in 1995 (Wave I) 1996 II) National Longitudinal Study Adolescent Health Public Use Dataset.
Main Outcome Measures
Abuse was assessed using a 5-point scale measuring if subject had been insulted public, sworn at, threatened with violence, pushed or shoved, something...
Abstract BACKGROUND: The authors conducted a randomized clinical trial examining the effects of modafinil in reducing persistent fatigue patients after treatment for cancer and performed secondary analyses to assess effect on cognitive function. METHODS: Breast who reported score ≥2 Brief Fatigue Inventory were enrolled study. In phase 1 (P1), received 200 mg open‐label once daily 4 weeks. 2 (P2), with positive response P1 either an additional weeks or placebo. Tests memory attention...

Objective
To evaluate the impact of School-Based Asthma Therapy trial on asthma symptoms among urban children with persistent asthma.
Design
Randomized trial, stratified by smoke exposure in home and randomized to a school-based care group or usual control group.
Setting
Rochester, New York.
Participants
Children aged 3 10 years
Interventions
Directly observed administration daily preventive medications school nurses (with dose adjustments...
