本研究の目的は, 外側ウェッジを片麻痺疹患者の足底に用いることで, 歩行時の動的な荷重・姿勢の非対称性が改善するかを予備的に検討することである. 片麻痺患者6名を対象に, 外側ウェッジ非装着時と装着一週間後に, 歩行時の垂直床反力の力積・単脚支持時間・体幹の側方動揺を測定した. その結果, 力積・単脚支持時間とも有意に増加し, 荷重対称性の改善を認めた. 体幹の側方動揺は有意に減少し, 姿勢の非対称性に改善を認めた. 外側ウェッジの使用は, 片麻痺患者の歩行時の下肢荷重の非対称性を改善する可能性があり, 無作為化比較研究を行う必要がある.

要旨:若年脳卒中患者の社会参加,特に復職は重要なリハビリテーションの目標であり,ノーマライゼイションの理念を具現化するものである.国際生活機能分類の普及に伴い,社会参加の重要性が再認識されている.また,政府が主導している「働き方改革」に関連した「治療と就労の両立支援」施策の一つとして,脳卒中の就労支援が進められている.しかし,脳卒中患者の高齢化・重度化,非正規雇用労働などの労働態様の変化は脳卒中患者の復職に多大な影響を与えており,若年脳卒中患者の復職率は過去20 年間,40%に留まっている.脳卒中患者の復職は医療だけでなく福祉分野とも関連し,職業リハビリテーションとの連携,さらには,復職予定先の企業等との調整など様々なレベルでの対応が必要であり,医療福祉連携を超える高次の連携が必要となる.
経頭蓋直流電気刺激(tDCS)は微弱電流により非侵襲的に大脳皮質神経細胞を刺激することで,脳卒中などの中枢神経障害による運動機能障害や神経心理学的障害に対して用いられる.片麻痺上肢をターゲットとする場合,陽極と陰極のパッド電極(5 cm×7 cm)を頭皮上の運動野直上と対側の眼窩上に置き,1~2 mA の微弱電流を10~20 分間通電する.刺激パラメーターは,電流の極性,強度および刺激時間によって規定される.経頭蓋磁気刺激と比較して刺激の空間分解能および時間分解能が低いが,安全性が高く,機器が比較的安価で運動中などでも使用できること,tDCS と他の運動療法とを組み合わせた様々な併用療法への応用が期待されている.本稿ではtDCS の併用方法として,上肢ロボット補助訓練および随意介助型電気刺激を紹介した.
高齢化社会は日本を筆頭に世界的に進み,働く高齢者が増えエイジフリーな職場環境が求められ,医療現場でもその一策として上肢訓練支援ロボットの開発が進み,最近は手指型が注目されている.機能訓練を十分量,反復して安全に行うことができ,作業療法の併用療法として効果のエビデンスは蓄積され,巧緻運動訓練も一部可能となった.治療として日本で利用できる環境は整いつつあるが,介入時間に制約がある中で簡便性,操作性,費用対効果の問題は避けて通れない.ロボットを使用しながら改良し進化を続ける時期であり,将来的には機能改善を巧緻性につなげる役割も期待され,上肢リハビリテーションの大きなブレイクスルーが得られる可能性もある.
