門脈下大静脈吻合ラットに酢酸アンモニウム(Am)を負荷することにより,シャント型肝性脳症の実験モデルを作製した.Am負荷30分後には半数が昏睡状態に陥ったが,2時間以内に全例が回復した.形態学的には,Am負荷30分後,アストロサイト(Ast)の胞体,ならびに突起の著明な腫大,細胞内小器官の増加,および核クロマチンの凝集を認めた.これらの変化は線条体,大脳皮質に著明であり,その程度は臨床症状の強さと一致した.Am負荷2時間後にはAstの腫脹は僅かに残存するのみであった.24時間後,昏睡時間の延長した2匹で,線条体,大脳皮質を中心に神経細胞の変性,壊死像を散見した.血中Am値は臨床症状の変化に一致した変動を示したが,Fischer比に有意な変化はなかった.以上より,本実験においては,Am負荷後,脳内Am処理のためAstが“活性状態”となり,一連の形態学的変化を示し,それに対応した可逆性の神経症状が出現するものと考えた.
