80歳以下で,併存症が軽度であり,かつ薬剤で除痛が得られる上部消化管穿孔に対して保存的治療を行い,穿孔部閉鎖後に胃癌の確定診断を得た症例を7例経験した.1991年から2011年の間に当院に入院した胃癌穿孔症例13例中,7例(53.8%)に保存的治療を行った.全例,保存的に穿孔部は閉鎖して,待機的に開腹した.6例に根治術を施行し,うち5例に5年生存を見た(Stage IIA1例,IIB1例,IIIA4例,以下,胃癌取扱い規約第15版に準拠).1例は局所進行のため非切除であり(Stage IVA),1年以内に癌死した.すなわち,保存的治療を選択した胃癌穿孔症例は,全例保存的に穿孔部閉鎖し,待機的に根治術を行って,比較的良好な予後が得られた.胃癌穿孔症例に対しても,一定の条件の下で初期治療として保存的治療は選択肢となると思われる.
