放射線照射後は組織の癒着や血流不全などの晩期影響により，周術期に出血や縫合不全などが起こりやすいことが知られている．われわれは34年前に卵巣腫瘍に対して全骨盤照射を行った患者に対して生体腎移植を施行した1例を経験した．術前検査のCTでは，吻合に使用する内腸骨動脈や外腸骨静脈の狭窄や壁肥厚および石灰化，周囲組織の癒着などの異常は指摘されなかった．生体腎移植施行後，血管吻合不全や血管吻合部狭窄などは認めなかったが，術後7日目に膀胱留置カテーテルを抜去後，移植尿管膀胱吻合不全による尿漏が生じたため，移植腎周囲へ経皮的ドレナージチューブを留置した．手術時に留置した移植尿管ステントに加え，膀胱に尿道カテーテルを長期間留置することで，吻合部からの尿漏は消失し，感染症などの合併症なく保存的に治療しえた．移植尿管膀胱吻合不全の治療として，尿路の減圧を行うことが，効果的だったことが考えられた．
