症例は68歳男性.急性発症した下腿浮腫を主訴に前医を受診し,高血圧,高血糖,低カリウム血症が確認された.血中ACTHとコルチゾールは高値を示しており,Cushing症候群疑いとして当院に紹介となった.胸部CTで右肺上葉に8 mmの結節を認め,ACTH産生腫瘍が強く疑われ,右上葉切除+リンパ節郭清(ND2a-2)を施行した.病理組織学的に定型肺カルチノイドと診断され,術後に血中ACTH値は著明に低下し,症状の改善が得られた.術後14ヵ月現在,再発なく経過している.
