症例は61歳,女性.貧血精査の上部内視鏡検査にて胃前庭部に圧排性病変を指摘された.造影CTにて胆嚢内を占拠する血流豊富な充実性病変を認めたが,明らかな漿膜および肝浸潤は認めなかった.胆嚢癌を疑い手術予定であったが,外来待機中に胆嚢炎を発症し,準緊急にて開腹拡大胆嚢摘出術および肝門部リンパ節郭清術を施行した.病理は広範な出血・壊死を伴う高異型度intracholecystic papillary neoplasm(ICPN)で,胆嚢管断端に腫瘍浸潤はなく,リンパ節転移も認めなかったため,追加切除や補助化学療法は施行しなかった.術後4カ月目,腹痛精査の造影CTにて腫瘍内出血を伴う多発肝転移を認めた.経カテーテル動脈塞栓術(以下TAEと略)にて状態が安定し,一旦退院としたが,TAEの1カ月後に再度腹痛をきたし,造影CTにて多発肝転移の増大と再度の腫瘍内出血を認めた.改めてTAEにて止血を行うも,その1カ月後(術後6カ月目)に癌死した.
症例は59歳男性で,心窩部痛を主訴に近医を受診し,膵頭部腫瘍の診断で紹介となった.腹部超音波で膵頭部に境界明瞭な径5 cm大の高エコー腫瘤を認め,内部に石灰化を伴っていたが,明らかな嚢胞成分は認めなかった.腫瘍は造影CTで遅延性の造影効果を示し,内部に変性を示唆する不染域を認めた.充実性のsolid-pseudopapillary tumor(SPT)あるいは乏血性の膵内分泌腫瘍の診断にて,亜全胃温存膵頭十二指腸切除術(SSpPD)を施行した.切除標本は被膜を有する白色調の充実性腫瘍で,内部に明らかな嚢胞成分は認めなかった.病理組織学的には腫瘍細胞は細血管周囲に偽乳頭状の増殖を示し,免疫染色にてchromogranin A(-),synaptophysin(-),CD10(+)であり,膵SPTと診断した.また核分裂像や静脈侵襲,神経周囲浸潤を認め,組織学的に悪性と考えられた.
