Background and Aims The association between hygiene prevalence of autoimmune disease has been attributed in part to enteric helminth infection. A pilot study experimental infection with the hookworm Necator americanus was undertaken among a group otherwise healthy people celiac test potential suppress immunopathology induced by gluten. Methods In 21-week, double-blinded, placebo-controlled study, we explored effects N. 20 healthy, helminth-naïve adults well controlled diet. Staged cutaneous...

The mucosal cytokine response of healthy humans to parasitic helminths has never been reported. We investigated the systemic and responses hookworm infection in experimentally infected, previously naive individuals from non-endemic areas. collected both peripheral blood duodenal biopsies assess immune response, as well at site adult worm establishment. Our results show that experimental leads a strong Th2 (IL-4, IL-5, IL-9 IL-13) regulatory (IL-10 TGF-β) with some evidence Th1 (IFN-γ IL-2)...

We present immunological data from two clinical trials where the effect of experimental human hookworm (Necator americanus) infection on pathology celiac disease was evaluated. found that basal production Interferon- (IFN-)γ and Interleukin- (IL-)17A duodenal biopsy culture suppressed in hookworm-infected participants compared to uninfected controls. Increased levels CD4+CD25+Foxp3+ cells circulation mucosa are associated with active disease. show this accumulation also occurs during a...

Summary Background Coeliac disease patients on a gluten‐free diet experience reactions to gluten, but these are not well characterised or understood. Systemic cytokine release was recently linked reactivation of gluten immunity in coeliac disease. Aim To define the nature and time‐course symptoms interleukin‐2 changes specific for patients. Methods 25 healthy volunteers consumed standardised 6 gram challenge. Disease Patient‐Reported Outcome survey global digestive symptom assessment were...

Summary Cytokines have been extensively studied in coeliac disease, but cytokine release related to exposure gluten and associated symptoms has only recently described. Prominent, early elevations serum interleukin (IL)-2 after support a central role for T cell activation the clinical reactions disease. The aim of this study was establish quantitative hierarchy cytokines their relation patients with disease during gluten-mediated reactions. Sera were analyzed from on free-diet (n = 25)...

Nexvax2® is a novel, peptide-based, epitope-specific immunotherapy intended to be administered by regular injections at dose levels that increase the threshold for clinical reactivity natural exposure gluten and ultimately restore tolerance in patients with celiac disease. Celiac disease fixed intradermal doses of Nexvax2 become unresponsive HLA-DQ2·5-restricted epitopes Nexvax2, but gastrointestinal symptoms cytokine release mimicking exposure, accompany first dose, limit maximum tolerated...

Background The prevalence and severity of duodenal injury in coeliac disease patients controlled on a gluten-free diet is unclear. Aims To use quantitative histology to assess treated disease. Methods Quantitative pre-treatment biopsies collected clinical trials assessing an investigational immunotherapy for was analysed. Morphometric readings were converted Marsh classifications. Results Ninety-three had biopsies. For well-oriented sections second part biopsies, six (6%) classified as 0 or...

Summary Background Nexvax2 contains three gluten‐derived peptides, intended to tolerize coeliac disease patients gluten. Sequences cover six epitopes that trigger immune activation in human leucocyte antigen‐DQ2.5‐positive patients, most notably after an initial dose. Patients experience gastrointestinal symptoms with increases serum interleukin‐2. Consistent Nexvax2’s induction of non‐responsiveness, reactivity disappears repeated doses, or is avoided gradual dose escalation. Early clinical...

The past decade has seen human leukocyte antigen (HLA) typing emerge as a remarkably popular test for the diagnostic work-up of coeliac disease with high patient acceptance. Although limited in its positive predictive value disease, strong association specific HLA genes imparts exceptional negative to typing, enabling result exclude confidently. Summary Background In patients with coeliac disease, FODMAPs in gluten‐containing foods, and participant anticipation of a harmful (‘nocebo’) effect, may contribute to acute symptoms after gluten challenge. Coeliac disease(CD) is a highly prevalent, gluten-dependent, autoimmune enteropathy. While the diagnosis based on serological and histological criteria, genotyping of human leucocyte antigens (HLA) DQ2 DQ8 has been shown to have substantial clinical utility, especially in excluding patients who do not carry either antigen. As result, HLA now being performed by more laboratories recently become one most frequently requested genetic tests Australia. To date, there little scrutiny accuracy...
