今回われわれは腸重積症にて発見された若年者大腸癌の1例を経験したので，文献的考察を加え報告する．症例は29歳，男性．腹痛と粘血便あり外来受診した．CT検査施行した結果，下行結腸に腸重積を認めた．高圧浣腸にて整復を行ったところ，腫瘤を先進部とした腸重積であった．大腸内視鏡検査では2/3周性の1型大腸癌を認めscopeはかろうじて通過したが内腔の狭小化を認めた．生検の結果moderately differentiated adenocarcinomaの診断にて腹腔鏡補助下結腸左半切除術を施行した．切除標本の病理結果は，SS INFβ ly1 v0 N0であった．自験例では大腸癌家族歴はなかったが，若年発症のためHNPCCの可能性もあり，MSI (microsatellite instability）解析を行ったが陰性であった．若年者においても腸重積を認めた場合，大腸癌の存在を念頭に置くべきと考えられた．

症例は52歳，男性．食欲不振，腹部膨満感を主訴に当院を受診した．上部消化管内視鏡検査で下部食道に3cm大の2型の食道癌を認め，さらに腹部CT検査で膵体部に接する75×75mm大の腫瘍を認めた．腹部血管造影では，脾動静脈が腫瘍により圧排狭窄を受けていた．孤立性で巨大な胃小弯リンパ節転移を伴う進行下部食道癌を疑い，手術を行った．胃小弯に位置する腫瘍は膵臓と一塊となり，脾動脈根部は目視できず，腹腔動脈根部で切離し，Appleby手術とした．右開胸開腹の胸部食道亜全摘，膵体尾部脾合併切除を行い，再建は大弯側胃管を用い，後縦郭経路にて胸腔内で吻合した．術後1週間目に縫合不全をきたしたが，保存的治療にて治癒し，術後7年経過するが無再発生存中である．食道癌でAppleby手術を施行することは稀であり，特に再建胃管の血流が最も危惧されるところであり，若干の考察を加え報告する．
