Hospitalized patients with SARS-CoV-2 infection (COVID-19) often receive antibiotics for suspected bacterial coinfection. We estimated the incidence of coinfection and secondary in COVID-19 using clinical diagnoses to determine how frequently are administered when is absent. performed a retrospective cohort study inpatients present on admission hospitals Premier Healthcare Database between April June 2020. Bacterial infections were defined ICD-10-CM diagnosis codes associated "present...
Transient acquisition of methicillin-resistant
Abstract We describe the epidemiologic and microbiologic characteristics of patients co-colonized with different species carbapenem-resistant Enterobacteriaceae (CRE) from 5 hospitals in 4 states. Twenty-eight 313 (8.9%) were at least 2 CRE species. Different within same patient showed identical mechanism resistance 18/28 (64%) cases.
Carbapenem-resistant Klebsiella pneumoniae (CRKp) is a significant public health concern that has contributed to the spread of carbapenem resistance and been linked high morbidity mortality. Transmission K. among patients through interactions with shared care personnel (HCP) described as source infection in settings; however, it remains unknown whether particular bacterial characteristics are associated increased CRKp transmission.
The impact of environmental cleaning protocol featuring PX-UV in reducing the incidence multidrug-resistant gram-negative
Background: Inappropriate antibiotic prescription leads to increased Clostridiodes difficile infections, adverse effects including organ toxicity, and generation of antibiotic-resistant bacteria. Despite efforts improve use in acute-care settings, unnecessary inappropriate still occur 30%–50% patients. Objectives: We assessed factors associated with at 2 time points: (1) initial, empiric therapy (2) 3–5 days after initiation. Methods: As part a multicenter study investigating strategies...
Abstract Objective Limited research of how to best taper opioids brings about an ethical and clinical dilemma. Experiments using overt concealed administration have demonstrated the benefits a reduction eliminate negative expectations prolong analgesic benefits. This may allow for opioid tapering without significant increases in pain. Based on this, we investigated patient provider acceptance chronic Methods We conducted cross-sectional survey via REDcap with 74 patients, who are currently...
Methicillin-resistant Staphylococcus aureus (MRSA) is a serious public health concern in the United States. Patients colonized and/or infected can transmit MRSA to healthcare workers and subsequent patients However, components of this transmission chain are just becoming evident, including certain patient factors, specific patient-healthcare worker interactions, microbial factors. We conducted comparative genomic analysis 388 isolates from four hospitals three states: Maryland, California,...
Abstract Frequent repositioning and skin examinations are the cornerstone of many pressure injury prevention programs. This study explores hypothesis that frequent skin‐exposing body care activities protective against injuries in residents long‐term facilities. We designed a cross‐sectional observational cohort 381 from 13 such Data were collected on resident characteristics nursing for each resident. analyzed those data, looking an association between presence stage 1 or greater....
Abstract In this cross-sectional study, we examined the relationship between resident level of care in nursing home and colonization with resistant gram-negative bacteria. Residential-care residents were more likely to be colonized bacteria than postacute (odds ratio, 2.3; 95% confidence interval, 1.40–3.80; P < .001).
In the coronavirus disease 2019 (COVID-19) pandemic, correctional facilities are potential hotspots for transmission. We examined genomic epidemiology of severe acute respiratory syndrome 2 (SARS-CoV-2) early in pandemic one country's largest urban jails. Existing SARS-CoV-2 isolates from 131 detainees at Cook County Jail Chicago, Illinois, March 2020 through May were analyzed by whole-genome sequencing. Contemporaneous Rush University Medical Center (Chicago, Illinois) and Global Initiative...
Abstract Background: Bright-light treatment is a safe and effective for the management of winter seasonal affective disorder (SAD). In recent study, we found that relative duration reading was positively associated with likelihood remission after six weeks light treatment. Methods: Two technicians measured illuminance box meter directed towards center material placed on table in front box. The measurement also performed removed. two measurements were randomized order. Friedman analysis...
Background: In October 2013, the University of Maryland Medical Center established a formal antibiotic prophylaxis protocol for patients undergoing ventricular assist device (VAD) placement, replacing previous system various broad-spectrum combinations typically prolonged durations based on surgeon preference. This new consisted standardized regimen 72 hours vancomycin and ceftriaxone after procedure. The objective this project was to evaluate rate surgical site infection (SSI) related VAD...
Background: As carbapenem-resistant Enterobacteriaceae (CRE) prevalence increases in the United States, risk of cocolonization with multiple CRE may also be increasing, unknown clinical and epidemiological significance. In this study, we aimed to describe epidemiologic microbiologic characteristics inpatients cocolonized CRE. Methods: We conducted a secondary analysis large, multicenter prospective cohort study evaluating factors for transmission healthcare personnel gown gloves. Patients...
Background: Healthcare personnel (HCP) acquire MRSA on their gown and gloves during routine care activities for patients who are colonized or infected with at a rate of ∼15%. Certain (eg, physical exam, endotracheal tube, wound bathing/hygiene) have been associated higher frequency transmission from the patient to HCP than other (ie, administration oral medicines, glucose monitoring, manipulation IV tubing/medication delivery). However, quantification contamination risk subsequent is poorly...
