The mechanisms of chronic atrial fibrillation (AF) are not well understood. We performed epicardial mapping AF in patients undergoing open heart surgery to test the hypothesis that is due a left "driver" with regular, short cycle length, resulting fibrillatory conduction rest atria.Nine (1 month >15 years' duration) were studied at surgery, 8 before and 1 during cardiopulmonary bypass. During AF, electrograms (AEGs) recorded for 5 minutes from 404 electrodes arranged bipoles along ECG lead...

Background— The mechanism(s) of persistent and long-standing (LSP) atrial fibrillation (AF) is/are poorly understood. We performed high-density, simultaneous, biatrial, epicardial mapping LSP AF in patients undergoing open heart surgery (1) to test the hypothesis that are due ≥1 drivers, either focal or reentrant, (2) characterize associated activation. Methods Results— Twelve with (1 month 9 years duration) were studied at surgery. During AF, electrograms recorded from both atria...

The risk for stroke after transcatheter aortic valve replacement (TAVR) is an important concern. Identification of predictors likely to be a critical factor aiding patient selection and management as TAVR use becomes widespread.Patients enrolled in the CoreValve US Extreme Risk High Pivotal Trials or Continued Access Study treated with self-expanding bioprosthesis were included this analysis. 1-year rate was 8.4%. Analysis hazard identified early phase (0-10 days; 4.1% strokes) late (11-365...

Whole-body arterial and venous dilating properties of nitroprusside nitroglycerin were compared with each other in 20 adult patients during cardiopulmonary bypass for coronary artery or valvular heart surgery simultaneous forearm vascular measurements. Nitroprusside was found to be a more effective whole-body vasodilator better when drug infused at rates 1.5 2.0 microgram/kg/min. No difference vasodilation between the two drugs 1.0 Forearm plethysmography did not distinguish any dose level.

Abstract Objective Determine the feasibility and predictors of early discharge after minimalist transcatheter aortic valve replacement (TAVR). Background Duration hospitalization has a direct impact on overall cost care, but clinical length stay (LOS) in patients undergoing TAVR remains unclear. Methods We studied 268 who underwent TAVR. Short LOS (sLOS) was defined as post‐procedural ≤ 3 days observed 163 patients. Prolonged (pLOS) 105 Propensity score matching based 39 variables yielded 54...

We previously demonstrated that persistent and long-standing atrial fibrillation is maintained by activation emanating from foci breakthrough sites of different cycle lengths (CLs). The purpose this study was to characterize the behavior focal nonrandom identified during high-density mapping in these patients.During open heart surgery, we recorded both atria simultaneously using 510 512 epicardial electrodes along with ECG lead II 12 patients fibrillation. For each patient, analysis 32...
