In Brief Background: Consensus on the definition of a Gustilo grade IIIC injury was sought, and influence vascular injuries outcome severe open tibial fractures investigated. Methods: Three thousand three hundred fifty-one plastic orthopedic surgeons were sent questionnaires to determine their interpretation injuries. Notes patients with reconstructed at Frenchay orthoplastic center free tissue transfer between 2006 2010 reviewed. Data collected patient demographics integrity. Outcome...
This retrospective study reviews the demographics, morphology and management of paediatric hand fractures. Notes all children with injuries attending a plastic surgery trauma clinic over one-year period were reviewed. Non-bony excluded. A total 303 fractures in 283 patients included. Fracture incidence rose after age seven, peaking at 14 years age; 76% occurred males. Sporting accounted for 47% Physeal present 39% cases. Open only 4% Management was primarily conservative. Only 5% cases...
Plastic surgery training in the UK continues to evolve towards an outcome-based rather than time-served curriculum. plastic trainees are appointed nationally, and assessed according national standards, but is delivered regionally. This study sought opinion from current order highlight strengths shortcomings of higher surgical programme.A cross-sectional was designed administered by Surgery Trainees Association (PLASTA). A questionnaire distributed all holding a National Training Number,...
Fractures of the metacarpals and phalanges are common. Placement Kirschner wires (K-wires) is most common form surgical fixation. After placement, a key decision whether to bury end K-wire or leave it protruding from skin (exposed). A recent systematic review found no evidence support either approach. The aim study was investigate current clinical practice, understand factors influencing clinician decision-making, explore patient preferences inform design randomized trial. The steering group...
Metacarpal fractures are common, accounting for 40 % of all hand injuries. The use plates the fixation these allows early aggressive therapy post-operatively, reducing post-operative stiffness. Traditionally, bicortical is standard practice, where both dorsal and palmar cortices metacarpal drilled through, with screws engaging cortices. Recent biomechanical studies have shown that unicortical fixation, only near cortex engaged by screw, results in no difference stiffness, load to failure or...
Medicine is becoming increasingly protocol driven. This provides a standardised format for doctors to deliver best practice, especially in the acute setting. 40 junior were asked locate three Trust guidelines: venous thromboembolism (VTE) prophylaxis; antibiotic prescribing; and management of upper gastrointestinal bleed (UGIB). For each doctor time taken number mouse clicks access guideline was recorded. Following successful redesign intranet we completed re-audit. Initial results showed...
Introduction Blunt chest wall trauma is commonly recognised as a significant source of acute pain following injury but the longer term outcomes in this patient cohort are less well described. The aim pilot study was to investigate incidence chronic patients with isolated blunt trauma, or without rib fractures. Methods A short survey completed over phone, postal survey. emergency department medical records and hospital database were used obtain demographic data, including age, pre-existing...
We report a previously well 9-year-old boy who presented with an acutely swollen left arm after horse riding. Left subclavian vein thrombosis was demonstrated by MR venography but there no evidence of underlying anatomical abnormality. The child successfully treated catheter directed thrombolysis and anticoagulation intravenous unfractionated heparin warfarin. have identified 75 published case reports effort-induced upper extremity deep (UEDVT) surveyed patient characteristics precipitating...
Introduction The improvements in short-term outcome after severe trauma achieved through early resuscitation and acute care can be offset over the following weeks by an systemic inflammatory response with immuneparesis leading to infection, multiorgan dysfunction/multiorgan failure (MOF) death. Serum levels of androgen precursor dehydroepiandrosterone (DHEA) its sulfate ester DHEAS, steroids immune-enhancing activity, are low traumatic injury at a time when patients catabolic...
Much research indicates that an individual's personality impacts the initiation and escalation of substance use problems in youth. The acquired-preparedness model suggests influences by modifying learning about substances, which then affects use. current study used longitudinal data to test whether automatic cannabis-related cognitions (memory associations outcome expectancy liking) mediate relationship between four traits with later cannabis use.The focused on a sample adolescents who had...

Aims
Combining the licensed Haemophilus influenzae type b (Hib) and serogroup C meningococcal (MenC) conjugate vaccine (Hib-MenC-TT) DTaP-IPV could reduce number of injections needed to administer United Kingdom routine infant immunisation schedule. We aimed demonstrate immunologic non-inferiority for Hib MenC using a 6-in-1 combination (DTPa-IPV/Hib-MenC-TT) compared schedule DTPa-IPV-Hib-TT MenC-CRM197 vaccines. Secondary objectives included demonstration diphtheria, tetanus polio...
