サルコペニアを有する血液透析患者の栄養状態の特徴と食事摂取状況について調査したので報告する. 血液透析患者90人を対象にサルコペニアのスクリーニング, 栄養スクリーニング, 食物摂取頻度調査を実施し, サルコペニアのリスク解析を行った. 対象者のうち22%がサルコペニアに該当し, サルコペニア患者の10~50%に栄養障害のリスクが認められた. 年齢とたんぱく質摂取量不足がサルコペニアの有意なリスク因子であった. サルコペニア群は非サルコペニア群より豆類, 魚介類・肉類, 菓子類・嗜好飲料・砂糖類の摂取量が有意に低かった. サルコペニアの予防には, 十分なたんぱく質摂取が必要であるが, 低栄養に関連するサルコペニアは10~50%であり, 半数以上が低栄養に関係なく筋肉量と筋力が低下していた. 現在透析患者に用いられている栄養評価には, 筋力や身体機能を含めた評価はほとんどない. 今後は運動習慣を含めた調査が必要である.
