目的: 探讨甲状腺透明变梁状肿瘤(HTT)的细针穿刺细胞病理学特征。 方法: 对5例HTT的临床、超声和细针穿刺细胞学形态进行观察,其中1例制备细胞蜡块,并行免疫组织化学检测。2例使用即时荧光定量PCR法进行BRAF V600E基因突变检测。 结果: 5例患者中女性4例,男性1例,发病年龄25~48岁。细针穿刺细胞涂片中可见肿瘤细胞呈放射状、梁状或巢状结构,肿瘤细胞与红染的玻璃样物质混杂。部分细胞胞质呈丝状,偶见黄色小体,细胞核圆形、卵圆形或梭形,可见多量核沟和核内假包涵体。1例细胞蜡块显示肿瘤细胞呈梁状结构,与玻璃样物质混合,细胞特征与涂片类似。Ki-67(MIB-1)免疫细胞化学染色肿瘤细胞呈特征性的膜质着色。2例使用即时荧光定量PCR法检测,均未发现BRAF V600E基因突变。 结论: HTT是一种罕见的甲状腺肿瘤,细胞形态与其他甲状腺肿瘤有重叠,在细针穿刺细胞病理诊断中非常容易被误诊。正确诊断的关键在于提高对该肿瘤的细胞病理形态的认识,制备细胞蜡块并行免疫组织化学检测具有重要的辅助诊断作用。.
